Corrections

How The RX Index handles factual errors, outdated information, and material updates.

Accuracy matters at The RX Index. When we identify or are notified of a material factual error, misleading statement, or outdated information that could affect a reader's understanding of a topic, we review it and make corrections as appropriate.

Our goal is to correct meaningful errors clearly, promptly, and transparently.

How to Report an Error

If you believe a page on The RX Index contains inaccurate or outdated information, please email corrections@therxindex.com with:

  • The URL of the page
  • A description of the issue
  • The specific statement you believe is inaccurate or outdated
  • A supporting source, if available

Submitting a report does not guarantee that a change will be made, but all credible correction requests are reviewed.

How We Review Correction Requests

When we receive a correction request, we review the claim against the source materials used for the page and any additional relevant sources available at the time of review.

If we determine that a material factual error is present, we update the page accordingly.

Depending on the issue, this may include correcting:

  • Pricing or fee information
  • Provider policy descriptions
  • Medication or regulatory statements
  • Sourcing errors
  • Wording that could materially mislead a reader

How We Document Corrections

When a correction materially changes the meaning of a page, we may add a correction note to the page itself and/or record the change in the correction log below.

A correction note may include:

  • The date of the correction
  • What was changed
  • A brief explanation of why the correction was made

We do not generally publish correction notes for minor edits that do not materially affect meaning, such as formatting improvements, grammar changes, typo fixes, or small clarifications.

Updates vs. Corrections

Not every page change is a correction.

We regularly review and update pages to reflect new pricing, policy changes, medication availability changes, or other developments. Those routine maintenance changes may update a page's Last Updated date without being listed as a formal correction.

We use the correction process for changes involving material factual accuracy.

Editorial Responsibility
