The Trulicity price in 2026 is $1,006.93 a month at list price, but few people pay that. Paying cash through Lilly's LillyDirect pharmacy starts at $389 a month at every strength. With commercial insurance that covers it, Lilly's savings card can bring Trulicity down to $25. With Medicare or Medicaid, your plan sets the price.
Below, we show what Trulicity costs in each situation, when paying cash can beat your insurance, how Medicare's 2026 cap works, and how to get it free if you qualify.

Decision Verdict
If your commercial plan covers Trulicity, use Lilly's savings card first. If you have no coverage, LillyDirect's $389 cash price is Lilly's lowest posted price. If you have Medicare, the 2026 Part D cap limits what you can pay in a year.
- Best for the savings card: Adults with employer or private insurance that covers Trulicity.
- Best for LillyDirect cash pay: People with no coverage for Trulicity, or whose plan would charge more than about $539 for a month before the savings card.
- Not ideal for either: Anyone on Medicare, Medicaid, TRICARE or VA coverage, who can't use the savings card. Check your plan's price and Lilly Cares instead.
- The factor that matters most: Your coverage type. It decides which discounts you're allowed to use.
How much does Trulicity cost? Price by situation
| Your situation | What you'd pay | What to know |
|---|---|---|
| No insurance, or Trulicity isn't covered | Starting at $389 a month through LillyDirect | Any strength. Prescription required. Taxes and fees may apply |
| Commercial insurance that covers Trulicity | As little as $25 for up to a 3-month fill with Lilly's savings card | Card saves up to $150 a month and $1,950 a year. Ends December 31, 2026 |
| Medicare Part D | Set by your plan. Lilly says about 70% of its Medicare patients paid $0 per fill in 2024 | Savings card not allowed. Part D out-of-pocket costs are capped at $2,100 in 2026 |
| Medicaid | Set by your state plan. Lilly says about 90% of its Medicaid patients paid $0 per fill in 2024 | Savings card not allowed |
| Low income, no coverage | Possibly free for up to 12 months through Lilly Cares | For people who qualify. Call 1-800-545-6962 |
| List price (before any discount) | $1,006.93 a month | Lilly's wholesale acquisition cost, as of January 1, 2026 |
Lilly's percentages come from its own analysis of 2024 pharmacy data. It also says about 40% of patients with employer or private insurance paid $0 per prescription and 90% paid under $75. Only your plan or pharmacy can tell you your exact price.
How much is Trulicity without insurance?
Paying cash, Trulicity starts at $389 a month through LillyDirect, Lilly's own pharmacy service. Lilly shows the same starting price for all four strengths: 0.75 mg, 1.5 mg, 3 mg and 4.5 mg. Your prescriber sends the prescription to LillyDirect Pharmacy, and the medicine can be delivered or picked up at a local pharmacy.
The government's TrumpRx site lists Trulicity at an "original price" of $987.19 with $598.19 in savings, which works out to the same $389. TrumpRx sends orders through LillyDirect.
What a year of cash pay costs (our math): Twelve months at $389 is $4,668. Each Trulicity box holds 4 weekly pens, and Lilly's savings card treats a month as 4 pens. If $389 buys one box, a full 52 weeks takes 13 boxes, or $5,057.
Two things to know before you pay cash:
- Cash purchases are made outside your insurance. LillyDirect's self-pay pharmacy partner has you agree not to seek reimbursement from any insurance plan, so don't expect a cash fill to count toward your deductible. Ask your plan if that matters to you.
- There's no cheaper generic. FDA's drug directory lists no dulaglutide sold in the US other than Lilly's Trulicity.
Trulicity price with insurance and the savings card
If you have employer or private insurance that covers Trulicity, Lilly's savings card can lower your cost to as little as $25 for a 1-, 2- or 3-month fill. The card has limits:
| Savings card rule | Detail |
|---|---|
| Most it saves per fill | $150 for 1 month, $300 for 2 months, $450 for 3 months |
| Most it saves per year | $1,950, and up to 13 fills |
| One month means | 28 days and up to 4 pens |
| Who can't use it | Anyone on Medicare (including Part D and Medicare Advantage), Medicaid, TRICARE, VA, DoD or a state drug program |
| Other limits | Ages 18 and older, US or Puerto Rico residents. Can't be combined with other discounts. Not allowed if your plan uses an alternate funding program |
| Ends | December 31, 2026 |
How the cap works (our examples): If your plan charges $100 for a month, the card covers $75 and you pay $25. If your plan charges $500, as can happen before you meet a deductible, the card covers $150 and you pay $350.
When cash can beat your insurance: With the card, you pay your plan's price minus up to $150. So once your plan charges more than about $539 for a month, LillyDirect's $389 cash price is cheaper for that fill. The catch is that a cash fill bought outside your insurance usually won't count toward your deductible, which can delay when your coverage kicks in. Ask your pharmacy for your plan's price before you choose.
Trulicity price with Medicare
Medicare doesn't let you use Lilly's savings card, but your yearly costs are capped. Under Medicare's 2026 rules:
- Your Part D deductible can't be more than $615 in 2026 ($700 in 2027).
- Once your out-of-pocket spending on covered Part D drugs reaches $2,100 in 2026 ($2,400 in 2027), you pay $0 for covered drugs for the rest of the year.
- The Medicare Prescription Payment Plan can spread your costs across the year. Medicare says it doesn't lower what you pay.
Your monthly price depends on whether your plan covers Trulicity and which tier it's on, so check your plan's drug list. For why savings cards are off-limits on Medicare, see why Medicare patients can't use GLP-1 savings cards.
Can you get Trulicity for free?
Possibly. The Lilly Cares Foundation, a nonprofit, gives prescribed Lilly medicines free for up to 12 months to US patients who qualify based on financial need. Lilly Cares says it's again taking new Trulicity applications, and these no longer need a medical exception request. Apply at lillycares.com or call 1-800-545-6962.
Does the Trulicity price change by dose?
Not on LillyDirect. The $389 starting cash price is the same for 0.75 mg, 1.5 mg, 3 mg and 4.5 mg. The list price is also one figure, $1,006.93 a month.
