On every commercial document we pulled, Mounjaro appears as Tier 2 (preferred brand) with prior authorization and a quantity limit. One public OptumRx Medicare formulary also shows Mounjaro at Tier 2 with QL of 2 mL per 28 days. Zepbound’s status varies more — covered on some plans, excluded on others.

Mounjaro is consistently coverable — but never automatically.

Every formulary we pulled listed Mounjaro at Tier 2 with prior authorization required. There's no OptumRx plan we found where you can walk in, hand over your card, and get Mounjaro filled without PA. The PA is the gate.

Quantity limits are real and dose-specific.

The public OptumRx Medicare example shows Mounjaro QL at 2 mL per 28 days — which equals four 0.5 mL pens, one per week, exactly the FDA-labeled dose. If you try to escalate your dose faster than 4 weeks at a level, or refill early because you traveled, the QL can trigger a rejection at the pharmacy counter even when your PA is approved.

Don't assume Mounjaro and Zepbound have the same coverage.

They're the same molecule (tirzepatide), made by the same company (Eli Lilly), but OptumRx treats them as completely different drugs because their FDA indications are different. We saw one plan where Mounjaro was Tier 2 and Zepbound was fully excluded.

The plan-document caveat: Every OptumRx formulary we pulled includes some version of this: “Coverage is determined by your prescription medication benefit plan. When differences between this list and your benefit plan documents exist, please refer to the information included in your benefit plan documents.” A public OptumRx formulary tells you what OptumRx generally does. Your employer can carve things out, add step therapy, or exclude a drug class entirely.

What PA, QL, ST, and “E” mean on your OptumRx formulary

These four letters appear next to almost every GLP-1 on every OptumRx formulary. Insurance jargon is the problem, not you.

If Mounjaro shows up with “PA, QL” next to it — that’s the most common pattern, and it’s fully approvable for the right diagnosis. “PA, QL, ST” means you’ll also need to document failed prior GLP-1s. “E” means you’ll need to fight harder, switch drugs, or pay cash.

OptumRx prior authorization criteria for Mounjaro (from the 04/2026 form)

OptumRx requires PA for Mounjaro on essentially every plan. To get approved under the diabetes pathway, your prescriber has to document a type 2 diabetes diagnosis with one of four qualifying lab values, no concurrent GLP-1 use, no personal or family history of medullary thyroid cancer or MEN2, an attestation that you’re making lifestyle changes, and proof that you tried a preferred GLP-1 first.

The four lab values that qualify you (you only need ONE)

HbA1c ≥ 6.5%

Most flexible criterion. The form explicitly notes: if original HbA1c is unknown, or current HbA1c is controlled due to another current diabetic regimen, please include current regimen and current HbA1c.

Fasting plasma glucose ≥ 126 mg/dL

Two-hour plasma glucose ≥ 200 mg/dL during an oral glucose tolerance test

Practical tip most patients miss: if you’re already on a diabetes regimen that controlled your A1C below 6.5%, that doesn’t disqualify you. You can submit your earliest documented A1C (the one that established the diagnosis), along with your current regimen and current A1C. Many denials at this step happen because the prescriber only submits the current (controlled) A1C with no historical context.

This is the step-therapy gate. Preferred GLP-1s on the form: Ozempic, Victoza, Wegovy, Zepbound, Rybelsus, Saxenda, Soliqua, Trulicity, and Xultophy. Mounjaro is listed as non-preferred. Your prescriber has to list what you tried, how long, and why you stopped.

Your printable PA prep checklist

This is the single highest-leverage thing you can do before your prescriber submits the PA.

Bring to the appointment:

  • Your most recent A1C result
  • Your earliest documented A1C (or fasting glucose ≥126, 2-hour glucose ≥200, or random glucose ≥200 with symptoms)
  • A list of every diabetes medication you've ever taken, with start/stop dates and the reason you stopped each one
  • Documentation of any side effects or intolerance from prior GLP-1s (especially Ozempic, Trulicity, or Wegovy)
  • A note about your lifestyle changes — diet, exercise, weight management program
  • Confirmation that you have no personal or family history of MTC or MEN2
  • Your insurance card with OptumRx info visible

Ask your prescriber to:

  • Submit the PA electronically (CoverMyMeds or ePA), not by fax
  • Use ICD-10 code E11.x only if Type 2 diabetes is accurately documented in your medical record
  • Document the step-therapy history in the “Length of trial / Reason for discontinuation” fields
  • Attest to lifestyle counseling and the relevant lab values
  • Request expedited review if your A1C is dangerously high or your diabetes is poorly controlled

Will OptumRx cover Mounjaro for weight loss? The hard truth most pages get wrong

Answer: No.

The OptumRx GLP-1 Agonists PA Form (Rev 04/2026) lists exactly four medications under Obesity Management Therapy: Wegovy, Saxenda, generic liraglutide for Saxenda, and Zepbound. Mounjaro is not on that list. Even if you meet the BMI criteria, the form has no question that pairs Mounjaro with obesity. The PA will be denied at the criteria-mismatch level.

Why so many pages say “yes, via Zepbound criteria” — and why they’re wrong

The reasoning sounds plausible: Mounjaro and Zepbound contain the same active ingredient (tirzepatide), made by Eli Lilly, at the same doses. Writers assume the criteria transfer.

They don’t. The OptumRx PA form is structured by drug name. When the prescriber submits a PA for Mounjaro under Obesity Management Therapy, the form routes them to Question 11 — which explicitly lists only Saxenda, generic liraglutide, Wegovy, and Zepbound. Mounjaro is not in the list. The PA gets denied not because the patient doesn’t meet BMI criteria — they might — but because the drug-and-diagnosis combination has no approval pathway on the form.

The honest path forward if you want tirzepatide for weight loss

If you’ve been using Mounjaro off-label and OptumRx is denying it, the structurally cleaner answer is Zepbound. Same molecule. Same dosing. Same once-weekly injection. But Zepbound is FDA-approved for chronic weight management and obstructive sleep apnea, and OptumRx has a documented PA pathway for it.

To qualify for Zepbound on OptumRx, you’ll generally need:

  • BMI ≥ 30, OR BMI ≥ 27 with at least one weight-related comorbidity (hypertension, dyslipidemia, type 2 diabetes, coronary heart disease, MASH/NASH, or sleep apnea)
  • Documented participation in dietary and lifestyle changes
  • No concurrent use of another weight-loss medication
  • For some plans: step therapy through phentermine or a less expensive weight-loss medication first

One ethical line we need to draw: We’ve seen pages and forum posts suggest asking your prescriber to “just code it as diabetes” so OptumRx will pay for Mounjaro when you actually want it for weight loss. Don’t do that. Coding a diagnosis that isn’t in your medical record is insurance fraud, full stop. The honest path is the same molecule, the right label, and a PA your plan can actually approve.

Denied by OptumRx? Here’s the appeal path that actually works

A first-pass Mounjaro denial from OptumRx can be fixable when caused by missing documentation, step therapy history, or diagnosis mismatch. You have 180 days from the denial letter date to file an internal appeal under federal law. If the internal appeal fails, you have a right to a free external review by an Independent Review Organization. Open your denial letter, find the “Reason for denial,” and match it to the table below.

The internal appeal letter that works

Your appeal letter should do four things, in this order:

  1. Reference the denial letter directly. Include the claim number, denial date, and the exact denial reason quoted from the letter.
  2. Cite the OptumRx criteria you meet — quoted from your plan documents, not paraphrased.
  3. Attach the evidence the original PA missed. Lab results, chart notes, prior medication history with dates, prescriber letter of medical necessity.
  4. Request a peer-to-peer review if the denial was clinical. Your prescriber speaks directly to an OptumRx clinical pharmacist — this often overturns denials that look bulletproof on paper.

Most Part D plans have 30 days to decide an internal appeal (72 hours for expedited). Commercial timelines vary by plan.

When to escalate to external review

If your internal appeal is denied, the law gives you a right to an Independent Review Organization (IRO) external review at no cost to you. The IRO is independent of OptumRx. Most states have a 4-month deadline from the final internal denial. External review can be powerful when the denial is clinical and your documentation directly supports the plan’s criteria. It rarely overturns straight benefit exclusions.

Mounjaro through Medicare, Medicaid, CHAMPVA, and TRICARE

Medicare Part D (including UHC Medicare Advantage + OptumRx)

  • Mounjaro is generally covered for type 2 diabetes with prior authorization. One public 2026 OptumRx Medicare formulary example lists Mounjaro as Tier 2 with PA and QL of 2 mL per 28 days.
  • The verified 2026 federal number: Part D annual out-of-pocket threshold of $2,100 for covered drugs.
  • Mounjaro is NOT on the 2026 Medicare GLP-1 Bridge list (which covers Wegovy, Zepbound KwikPen, and Foundayo at $50/month from July 1, 2026).
  • Lilly Savings Card not available for Medicare patients — federal anti-kickback rules prohibit manufacturer copay assistance for government beneficiaries.
  • Medicare Prescription Payment Plan (M3P): Spreads covered out-of-pocket prescription drug costs across the calendar year. It does not lower the cost — it changes the timing. Free opt-in through any Part D plan.

Medicaid (state-dependent)

Medicaid coverage is state-dependent. In the TennCare OptumRx documents we reviewed, Mounjaro appears as a non-preferred GLP-1 with PA and QL. Almost no Medicaid program covers Mounjaro for weight loss. The most common denial pattern in public Medicaid appeal examples: diagnosis mismatch and missing documentation. CHAMPVA

OptumRx’s CHAMPVA page is explicit: Since January 1, 2025, CHAMPVA covers Mounjaro and Ozempic only with a confirmed type 2 diabetes diagnosis. The page specifically excludes coverage for prediabetes, weight loss, or obesity.

Current VA Meds by Mail language: Mounjaro is only covered with a type 2 diabetes diagnosis; Wegovy is only covered with a diagnosis for prevention of major adverse cardiovascular events; Zepbound is only covered with an obstructive sleep apnea diagnosis. Don’t treat CHAMPVA weight-loss GLP-1 coverage like a normal commercial-plan exception hunt.

TRICARE

TRICARE uses Express Scripts, not OptumRx, for pharmacy benefits. If you’re on TRICARE, the OptumRx rules on this page don’t apply directly to you.

If you have a government plan, do not start with a third-party provider service before checking your own plan’s coverage. Ro states its insurance concierge currently can’t help coordinate GLP-1 coverage for most government insurance plans (with an FEHB exception). Your first step: check your OptumRx member portal or member services for Mounjaro coverage; call the number on your member card and ask the specific PA criteria; have your existing prescriber submit the PA.

Mounjaro vs. Zepbound on OptumRx — they are not interchangeable for coverage

Mounjaro and Zepbound are the same molecule (tirzepatide), made by Eli Lilly, at the same doses. For OptumRx coverage purposes, they are completely different drugs. The PA forms route them differently. Asking for Mounjaro when you need the weight-loss path is the most preventable cause of denial.
