Does Humana cover Wegovy in 2026?

Quick answer: Sometimes — and the answer depends on three things working together: your specific Humana plan, the indication on the prescription, and which form of Wegovy your doctor prescribes. Humana commercial plans often cover Wegovy with prior authorization (though some employer plans exclude weight-loss drugs entirely). Humana Medicare plans do not cover it for weight loss alone today — but that changes for many members on July 1, 2026, when the Medicare GLP-1 Bridge launches.

Wegovy has three FDA-approved uses, and Humana treats them differently

The original approval. Most discussions about Wegovy coverage focus on this use.

March 2024

Cardiovascular risk reduction

Reducing the risk of major adverse cardiovascular events (heart attack, stroke, or cardiovascular death) in adults with established heart disease plus obesity or overweight. The SELECT trial showed a 20% drop in serious heart events versus placebo.

Current label

Noncirrhotic MASH (injection only)

Wegovy injection is approved to treat noncirrhotic MASH with moderate to advanced liver fibrosis in adults. Wegovy tablets are labeled for cardiovascular risk reduction and weight reduction, NOT MASH.

Why the indication matters on Medicare: Federal law from 2003 blocks Part D from paying for drugs prescribed only for weight loss. But the cardiovascular and MASH uses are separate FDA-approved indications that the law does not exclude. If you have Humana Medicare and established heart disease (or MASH with moderate-to-advanced fibrosis), your Part D plan may cover Wegovy under those indications today — even before the Bridge opens.

Commercial Humana plans (employer, marketplace, individual)

If you have commercial Humana — through your job, the marketplace, or as an individual policy — coverage of Wegovy is plan-specific. Many commercial plans cover it with prior authorization. Some employer plans carve weight-loss medications out entirely.

When covered, Wegovy is typically placed at Tier 3 (preferred brand) or Tier 4 (non-preferred brand). Prior authorization criteria typically include:

  • ·BMI of 30 or higher, OR BMI of 27 or higher with a weight-related health condition (high blood pressure, type 2 diabetes, high cholesterol, sleep apnea, or cardiovascular disease)
  • ·Documented attempts at lifestyle changes (diet and exercise), often for at least 3 to 6 months
  • ·A prescription written by a provider experienced in obesity management or endocrinology (preferred on some plans)
  • ·Confirmation that Wegovy will be used along with a reduced-calorie diet and increased physical activity
Critical wrinkle: If your employer carved weight-loss medications out of the plan, no provider can get Wegovy covered through Humana — no matter how good their insurance team is. That is a benefit design decision, not a medical necessity decision. Appeals against benefit exclusions almost never succeed.

Humana Medicare Advantage and standalone Part D plans

Today, Humana Medicare Advantage and standalone Part D plans do not cover Wegovy for weight loss alone. That is federal law, not a Humana policy choice. Two things change that picture:

TODAY — if you have CVD or MASH

Wegovy for cardiovascular risk reduction in adults with established heart disease, or for noncirrhotic MASH with moderate-to-advanced liver fibrosis (injection only). These are not “weight loss only” uses, so Medicare Part D plans can cover them when criteria are met. KFF estimated around 3.6 million Medicare beneficiaries have both obesity and cardiovascular disease — a real pathway for a lot of people.

JULY 1, 2026 — Medicare GLP-1 Bridge

A temporary CMS demonstration running through December 31, 2027. Eligible Part D and Medicare Advantage prescription drug plan members get access to Wegovy formulations, Zepbound KwikPen, and Foundayo at a flat $50/month copay. CMS picked Humana as the central processor for the Bridge for the entire country.

Humana coverage for MASH

Wegovy injection is FDA-labeled for noncirrhotic MASH with moderate-to-advanced liver fibrosis. Wegovy tablets are NOT on the MASH label. Coverage follows the same plan-specific PA path as the cardiovascular indication — confirm with your plan documents.

Humana Healthy Horizons (Medicaid)

Humana operates Medicaid managed care plans under Healthy Horizons in several states. Coverage of GLP-1s for obesity varies — only about 13 states cover them for that use at all. If your state does not cover them, an in-network PCP or cash-pay route is the realistic path.

FEHB administered by Humana

Why your Humana Wegovy claim might auto-reject at the pharmacy (the Reject Code 88 problem)

Quick answer: Since June 2023, Humana has used a pharmacy-level edit that requires an ICD-10 diagnosis code on certain claims. There are two specific reject codes you might see at the counter. Reject Code 39 means the claim is missing or has invalid ICD-10 information — easy fix, your doctor's office adds the code. Reject Code 88 means the submitted diagnosis code does not support a medically accepted indication for the drug under your plan. That is a different conversation.

Reject Code 39 — administrative fix

Missing or invalid ICD-10 diagnosis information on the claim. The fix is administrative — your prescriber's office adds or corrects the diagnosis code and the pharmacy resubmits.

Reject Code 88 — indication mismatch

The submitted ICD-10 code does not represent a medically accepted indication for the medication under the plan, or age criteria are not met. First, ask what diagnosis code was submitted and whether that indication is covered under your plan. The next move flows from that answer.

The legitimate paths to a covered indication on Wegovy with Humana

Cardiovascular risk reduction

Since March 2024, Wegovy has been FDA-approved to reduce major adverse cardiovascular events in adults with established CVD plus obesity or overweight. If you have a history of heart attack, stroke, or peripheral arterial disease, talk to your doctor about whether the prescription should be tied to the cardiovascular indication. The diagnosis codes and PA criteria are different, and the cardiovascular pathway is not blocked by the Medicare weight-loss exclusion.

MASH with moderate-to-advanced liver fibrosis

If your doctor has diagnosed noncirrhotic MASH with moderate to advanced fibrosis, Wegovy injection is FDA-labeled for that use. Wegovy tablets are not on the MASH label.

Type 2 diabetes — different brands, mostly

If the clinical goal is blood-sugar management for type 2 diabetes, Ozempic, Mounjaro, and Rybelsus are diabetes-labeled options that typically have stronger and more reliable coverage. Wegovy is labeled for weight reduction, cardiovascular risk reduction, and (for injection) MASH — not as the primary glycemic-control brand.

Sleep apnea — Zepbound, not Wegovy

Wegovy is not FDA-approved for obstructive sleep apnea. Zepbound has that approval (December 2024). If sleep apnea is your indication, ask your clinician whether Zepbound is the more appropriate pathway.

Who should not use an online Wegovy provider just because it accepts Humana

Online Wegovy programs are not right for everyone. If any of the following apply to you, please talk to your in-person doctor first before paying for an online program:

  • Personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). This is a direct FDA contraindication for Wegovy.
  • Known serious hypersensitivity (allergic reaction) to semaglutide or any Wegovy ingredient.
  • Pregnancy or planning pregnancy. The Wegovy label says it may cause fetal harm. Discontinue at least 2 months before a planned pregnancy because of semaglutide's long half-life.
  • History of pancreatitis, gallbladder disease, or severe gastrointestinal disease. These need clinical evaluation and ongoing monitoring most online programs are not built for.
  • Multiple interacting medications or complex diabetes regimens. Coordination matters more than convenience.
  • Type 1 diabetes. Wegovy is not studied for type 1 diabetes.
  • You are a minor under 18. Wegovy is approved for adolescents 12 and older for weight management, but the prescribing context for minors should be coordinated with a pediatric specialist, not an adult online program.
