Is Retatrutide on LillyDirect? Not Yet — Here's What Has to Happen First

By The RX Index Editorial Team · Last verified October 2026

No — LillyDirect does not sell retatrutide. It is still an investigational Eli Lilly drug, not FDA-approved or available for an ordinary pharmacy prescription. Lilly's stated filing target is early 2027, not a launch date. The two documented medical access paths are a qualifying Lilly trial and a narrow, doctor-requested expanded-access program.

This isn't a stock problem, and there's no restock button. Below is the dated 7-milestone tracker, what LillyDirect does sell today and what it really costs — and the catch most people miss: websites are already selling "Lilly" retatrutide, and none of them are Lilly.

RX Index Decision Verdict: Don't wait on LillyDirect for retatrutide if you need help this year.

  • Best for: people who want authentic Lilly medicine and need to check medical access, wait for approval, or discuss an approved Lilly option now.
  • Not ideal for: anyone hoping to order retatrutide today, from Lilly or anyone else.
  • The factor that matters most: FDA approval is required for a retail launch, but Lilly must also choose to offer retatrutide through LillyDirect.

No page, including this one, can tell you whether a prescriber will think a GLP-1 is right for you. A licensed clinician decides. Below, you can check the access rules and see what to ask your doctor.

LillyDirect retatrutide: the 7-milestone tracker

Lilly has reported Phase 3 results from five retatrutide trials; other trials are still underway. As of October 16, 2026, retatrutide has no approval, announced commercial price, or LillyDirect listing. We found no announcement that Lilly had submitted its application for U.S. marketing approval. These seven milestones show what is known—not a guaranteed launch schedule.

Think of LillyDirect as a pharmacy shelf. FDA approval is needed before a new medicine can go on that shelf, but Lilly still has to choose to put it there. Retatrutide hasn't reached the shelf yet.

The filing-path update: Lilly's September 29 release called its planned application a Biologics License Application, or BLA. But an FDA letter dated October 2, 2026, filed in court on October 8, says retatrutide is a drug rather than a biological product and must use the drug-application pathway under section 505. That letter is about which filing rules apply—not a decision to approve or reject retatrutide for sale. Early 2027 remains Lilly's stated target in the release, not a confirmed launch date.

The rows are not a fixed-order checklist. For example, Lilly could announce a price before approval, and it does not have to offer a savings program.

Two things worth knowing about that last row:

This is a dated snapshot, not a live feed. Use the linked official records for changes after October 16, 2026.

Retatrutide isn't a routine prescription option yet, but a GLP-1 that fits you might be. The right path depends on your state, your insurance and formulary (your plan's list of covered drugs), whether you want FDA-approved or compounded medicine, a shot or a pill, and your budget. The RX Index's Find My GLP-1 Path tool asks a few quick questions and suggests a GLP-1 path and provider to review. It's free, takes about 2 minutes, and needs no signup. It does not check your plan's coverage or match anyone to retatrutide.

Will LillyDirect sell retatrutide after FDA approval?

Lilly hasn't announced a retatrutide listing in the sources we checked. LillyDirect currently lists approved Lilly medicines, and Lilly announced Foundayo shipping through LillyDirect within days of its approval. So LillyDirect is a sensible place to watch, but its future retatrutide role is unconfirmed.

Here's what that means for you right now:

  • There is no LillyDirect retatrutide waitlist or pre-order in any of the Lilly sources we checked. If a site offers you a "spot in line" with Lilly's name on it, treat it as a red flag and use the check further down.
  • There's no launch date. Lilly's plan is to apply in early 2027. An application is a request, not an approval. For the broader access picture, see our retatrutide availability guide.
  • There's no price. Lilly's FAQ says only that it plans to price retatrutide to reflect its overall value. Any LillyDirect retatrutide price you see today is a guess. Our retatrutide cost predictions are clearly labeled forecasts, not Lilly prices.

Can my doctor send a retatrutide prescription to LillyDirect?

No. There's no approved retatrutide product for an ordinary retail prescription, so a pharmacy cannot fill one through LillyDirect. A telehealth visit or insurance coverage doesn't change that. Trials and expanded access use different, investigational arrangements.

Can I still get real retatrutide some other way?

The two documented medical access paths to authentic Lilly retatrutide are a trial and expanded access; neither is a LillyDirect purchase. Our October 16, 2026 ClinicalTrials.gov search for retatrutide and Lilly found one recruiting trial record and one available expanded-access record.

1. A clinical trial. The one Lilly retatrutide study listed as recruiting is SYNERGY-Outcomes (NCT07165028), last updated October 8, 2026. It's a Phase 3 study for adults with MASLD — fatty liver disease linked to metabolic problems — who are at higher risk of liver complications. It's not a general weight-loss study. Inside it, a retatrutide group is compared with a placebo group (substudy RT01), so joining doesn't guarantee you get retatrutide. To ask about it, use Lilly's trial finder and ask whether an accessible site is screening for the retatrutide RT01 substudy before you change any plans. A recruiting label on the overall study does not confirm an opening in that substudy at your site.

2. Expanded access. This is a narrow, doctor-requested exception for some adults with severe obesity. Lilly's public record (NCT07629401) requires all five of these:

Lilly's posted ruleIn plain words
Age 18 or olderAdults only
Refractory obesity: BMI of 35 kg/m² or more despite sticking with and tolerating the highest available dose of chronic weight-management therapyBMI (body mass index) is a number from your height and weight. "The highest dose I could handle" isn't the same as the highest available dose.
Two or more serious or life-threatening obesity-related conditions receiving standard care nowAt least two serious conditions under current treatment, not just past diagnoses
Can't join a retatrutide or comparable trial, because you don't qualify or no enrolling site is reasonably accessibleA trial isn't workable for you; access is not only a question of distance
All standard options, including weight-loss surgery, discussed with you through shared decision-makingA conversation with your clinician, not a requirement to have surgery

A medical reason not to take retatrutide rules a person out. Meeting the rules on paper still doesn't guarantee access — Lilly says availability depends on location, supply, and an individual review.

Does expanded access come through LillyDirect?

No. Your treating doctor starts the request by calling the Lilly Answers Center at 1-800-545-5979, not by sending a retail prescription to LillyDirect. Patients should speak with their own doctor rather than call Lilly to apply. According to Lilly Medical's expanded-access page (last reviewed August 26, 2026), Lilly provides the medicine at no cost but does not ship it directly to patients. The doctor must sign an agreement with Lilly, arrange FDA authorization and ethics-board review (an institutional review board, or IRB), oversee receipt, storage and dispensing, and report side effects.

"Free medicine" isn't the same as a free plan:

CostWhat Lilly's published terms sayAsk before you start
The retatrutide itselfProvided at no costIs anything about handling billed to me?
Doctor visits, labs, monitoringNot covered by that statementWhat will I owe, and how often?
Ethics review or paperworkNot covered by that statementWill any review fee be passed to me?
TravelNo terms foundWhere do I go, and how often?

An unknown cost isn't a $0 cost. Lilly also says expanded access will end after potential FDA approval and commercial availability, so ask how treatment and payment would continue then. For the full five-rule worksheet and a message you can bring to your doctor, see who qualifies for Lilly's retatrutide early access.

Is that "Lilly retatrutide" seller real? The quick check

No one can legally sell retatrutide to consumers right now — not Lilly, not a pharmacy, not a clinic. If a site claims you can buy retatrutide from LillyDirect, that claim is false.

The FDA says (checked October 16, 2026) that retatrutide cannot be used in compounding under federal law and has not been found safe and effective for any condition. It has warned telehealth companies, ingredient sellers, and outsourcing facilities over retatrutide. Lilly puts it plainly: "Anyone purporting to sell retatrutide to consumers is breaking the law" (Lilly).

Source: LillyDirect FAQ, checked October 16, 2026. A text from a partner pharmacy can be legitimate. When unsure, call LillyDirect at the number above to verify the message before paying. A listed number or ID alone does not prove a seller is real.

To check a marketed Lilly medicine you already have, Lilly offers a verification tool at scan.lilly.com. It is not a way to authenticate a retail retatrutide vial: no approved retail retatrutide product exists.

Why this matters:

  • Lilly is suing sellers. Lilly announced six lawsuits on August 12, 2026. October 8 court complaints also include cases against LiveWell Peptides and Medrein Health & Aesthetics. These are Lilly's allegations, not court findings. More in our Lilly retatrutide lawsuits guide.
  • Serious harm has been reported. In a June 19, 2026 alert, Victoria's health department in Australia reported six cases of acute liver injury since January in people using unapproved products labelled retatrutide. Officials said contaminants might be involved and the contents were still under investigation. These reports do not prove that authentic Lilly retatrutide caused the injuries.

Report a fake: call the Lilly Answers Center at 1-800-545-5979, file with FDA MedWatch, or contact your state Board of Pharmacy.

If you already used something labeled retatrutide and feel unwell: call your doctor and bring the vial and label. Call 911 if someone has trouble breathing, collapses, has a seizure, or cannot be awakened. For poisoning questions, call Poison Control at 1-800-222-1222.

For why there's no legal compounded version, see why retatrutide can't be compounded. For other warning signs, see how to avoid fake GLP-1 medications online.

What can you get from LillyDirect today?

LillyDirect lists 10 Lilly medicine brands or product families. For weight loss, that includes Zepbound, a weekly shot, and Foundayo, a daily pill. You need a prescription first—LillyDirect connects you with dispensing pharmacies; it does not make the prescribing decision.

The 10 listed brands or product families (checked Oct 16, 2026): Zepbound · Foundayo · Mounjaro · Trulicity · Emgality · Humalog family · Humulin family · Basaglar · Rezvoglar · Lyumjev. Retatrutide isn't on the list.

Already have an appropriate prescription? Start with Lilly's official Zepbound page or Foundayo page. You do not need to join a second care program just to use LillyDirect.

How Zepbound and retatrutide are related

Both are once-weekly Lilly injections, and both act on the GIP and GLP-1 hormone receptors. Retatrutide also acts on a third one, glucagon. Of those two drugs, only Zepbound (tirzepatide) is FDA-approved. Foundayo (orforglipron) is a different approved medicine again — it is not a retatrutide pill.

Sources: Zepbound prescribing information, Foundayo prescribing information, and Lilly's retatrutide information. Zepbound also has an approved use for moderate-to-severe obstructive sleep apnea in adults with obesity; the table above focuses on weight management.

For how retatrutide's trial results compare, see retatrutide vs. tirzepatide. If you're on Zepbound now and wondering about switching later, read switching from tirzepatide to retatrutide — and don't stop a prescribed medicine to chase a trial spot without your prescriber.

Safety note: Zepbound caused thyroid C-cell tumors in rats; it is not known whether it causes these tumors in humans. Don't use Zepbound or Foundayo if you or anyone in your family has had medullary thyroid carcinoma (MTC), or if you have MEN 2 (multiple endocrine neoplasia type 2). Their prescribing information also lists serious allergy restrictions and other safety warnings to review with your clinician.

LillyDirect self-pay prices

These are Lilly's cash prices when you pay without insurance. Checked October 16, 2026, on Lilly's Zepbound and Foundayo pages.

MedicineDoseLillyDirect self-payThe fine print
Zepbound (single-dose vial or KwikPen)2.5 mg$299 per 28-day fill1 KwikPen or 4 single-dose vials. Pen needles or syringes cost extra. This is a starting-dose price, not the ongoing maintenance price.
5 mg$399 per 28-day fillMedication only
7.5 mg$449 per 28-day fill with the Journey offer$499 without the offer; timing rule below
10, 12.5, or 15 mg$449 per 28-day fill with the Journey offer$699 without the offer; timing rule below
Foundayo (pill)0.8 mg$149 per 30-day fillStarting-dose price; higher doses can cost more
2.5 mg$149 per 30-day fillTemporary through Dec 31, 2026; published regular price $199
5.5 mg$199 per 30-day fillTemporary through Dec 31, 2026; published regular price $299
9 mg$299 per 30-day fillMedication only
14.5 or 17.2 mg$299 per 30-day fill with the Journey offer$349 without the offer; timing rule below

The 45-day rule starts when you receive the previous prescription—not when it was written. The Journey offers apply automatically to the first qualifying purchase at the listed higher doses. To keep the offer, complete the next refill purchase within 45 days of delivery or receipt of your previous prescription. This is a purchase rule, not a dosing schedule. Sources: Zepbound full terms and Foundayo full terms.

These are medication prices, not the cost of a doctor's visit or care plan. LillyDirect advertises free home delivery; injection supplies cost extra. Lilly's terms allow additional taxes or fees and changes to the offers. Check the final pharmacy total before paying.

Two other ways to pay:

  • Commercial insurance: with eligible coverage, Lilly's cards can reduce the price of the Zepbound single-dose pen or Foundayo to as little as $25 for a qualifying fill. Savings caps and other card rules apply; this is not the self-pay vial or KwikPen offer above. People enrolled in government plans, like Medicare or Medicaid, cannot use these commercial savings cards. Check the Foundayo rules for its separate limits.
  • Medicare Part D beneficiaries: CMS lists Foundayo and Zepbound KwikPen for eligible people through the temporary Medicare GLP-1 Bridge, which runs outside the usual Part D payment system. Zepbound single-dose vials and single-dose pens are not included. Eligibility and prior authorization still apply; an indication already coverable under Part D follows that plan instead. See our Medicare GLP-1 Bridge guide.

What a first year of Zepbound from LillyDirect could cost: 13 fills

The sticker price is one fill. The real cost is the medicine plus the care and supplies around it. Here's a 13-fill example at today's prices—not a dose plan or a complete treatment quote. Your prescriber sets the dose and any changes; 13 fills of 28 days cover 364 days, or 52 weeks.

Say a prescribed treatment plan results in one $299 fill, one $399 fill, then eleven fills in the $449 price tier, with each purchase meeting the offer rules:

Period or scenarioMedicine from LillyDirectAlso budget for
First 28-day fill$299A prescriber's visit or membership, any separate labs, injection supplies, and applicable taxes or fees
Second 28-day fill$399Care and supplies as needed
Fourth 28-day fill$449Care and supplies as needed
First 52 weeks (13 fills; 364 days)$5,637 ($299 + $399 + 11 × $449)Care and supplies across the year; not included in this medicine total
Alternative: one $299 fill, then twelve prescribed 5 mg fills at $399$5,087 ($299 + 12 × $399)Same excluded costs
If one 10, 12.5, or 15 mg refill misses the offer windowThat fill is $699, or $250 moreOther costs still apply

Why 13 fills? A year has about 13 four-week stretches, not 12. This example covers 364 days, not every day of a 365-day year. It also assumes these prices stay unchanged; it is not a price lock for the next year.

LillyDirect does not require a care membership to use its pharmacy service. But the prescriber is separate. LillyDirect lists independent telehealth options; for example, its directory shows Sesame at $99 a month or $59 a month on an annual plan, and WeightWatchers at $149 a month or $74 a month with a 12-month plan. Those are provider-reported rates and do not include medicine.

The payment schedule matters. Sesame's own billing FAQ says the $99 plan is billed every 28 days, despite calling it monthly. Its annual starting rate is $708 paid in advance for 12 months ($59 × 12); some providers charge more. WeightWatchers' linked offer currently says $25 for the first month, then $74 a month for the rest of a 12-month commitment, or $839 over that first term ($25 + 11 × $74), before medication. It renews for another 12-month term at $74 a month. A lower monthly figure can mean a longer commitment, not a smaller amount due today.

What we actually verified

We read the primary sources linked below on October 16, 2026, including Lilly's pages, FDA information, the FDA's court-filed letter, and ClinicalTrials.gov's structured records. Here's what we confirmed, where, and what we couldn't.

Last verified October 2026. Sources checked October 16, 2026:

What we found that you should know:

  • Lilly's general retatrutide FAQ still says retatrutide is legally available only through clinical trials. Lilly's more specific expanded-access page and the ClinicalTrials.gov record describe the expanded-access exception. We rely on the specific records for that exception.
  • Lilly's September filing announcement calls the planned application a BLA; the FDA's October 2 letter says retatrutide must use the drug-application pathway instead. We distinguish those dated positions rather than call the BLA route settled.
  • Both Ro and LillyDirect showed Foundayo 2.5 mg at $149 under the temporary offer on October 16. Ro's page also shows the old $199 price crossed out. Matching medication prices do not remove Ro's separate membership fee.

What we couldn't confirm: whether Lilly will sell retatrutide on LillyDirect, its commercial price or approval date, any patient's total expanded-access cost, or whether a particular trial site is screening this week. We did not submit patient information, complete a purchase, contact Lilly, or call a study site. Published offer terms can be checked; your final eligibility and checkout total cannot be promised here.

LillyDirect retatrutide FAQ

Is retatrutide FDA-approved?

No. Lilly says retatrutide remains investigational and has no regulatory approval. Its September 29 filing target was the first quarter of 2027. The later FDA letter changes the filing-path picture, not retatrutide's unapproved status.

Where can you get retatrutide?

The documented paths to authentic Lilly retatrutide are a qualifying Lilly trial or, for some adults with severe obesity, an expanded-access request started by their treating doctor. It is not an ordinary pharmacy purchase, including through LillyDirect, and online retail sellers are not an approved access path.

When will retatrutide be available?

No commercial launch date has been announced in the sources we checked. A U.S. launch needs FDA approval, and Lilly's published target is to apply in early 2027. That is a planned filing window, not a promised launch date.

How much will retatrutide cost on LillyDirect?

Lilly has not announced a commercial price, savings card, or LillyDirect offer in the sources checked. A forecast or an online seller's price is not an official Lilly retail price.

Is retatrutide just out of stock on LillyDirect?

No. This is an approval-and-access issue, not an ordinary stock shortage: retatrutide is not approved or on LillyDirect's current medicine list. There's nothing to restock, and no restock alert will get you into a trial or expanded access.

Will LillyDirect sell retatrutide?

No retatrutide listing has been announced in the sources we checked. For Foundayo, Lilly announced immediate prescription acceptance and shipping beginning five days after approval, so LillyDirect is worth watching after an approval. That's history, not a promise.

Can I pre-order or join a LillyDirect waitlist for retatrutide?

We found no official waitlist or pre-order in any Lilly source we checked. Treat any "reserve your spot" offer with Lilly's name on it as a red flag. Lilly's expanded-access program is a doctor's request, not a line you can join.

Is a website selling "Lilly retatrutide" real?

No. Lilly says anyone selling retatrutide to consumers is breaking the law. Real LillyDirect lives at lilly.com/lillydirect, requires a prescription, and doesn't offer retatrutide.

Can my doctor prescribe retatrutide and send it to LillyDirect?

No. There's no approved retatrutide product for a prescription to fill. A doctor's role right now is to look at trials or an expanded-access request with you.

Is expanded-access retatrutide free, and does it come through LillyDirect?

Lilly says it provides the medicine at no cost, but visits, tests, monitoring, review fees, and travel may still cost you. It does not come through LillyDirect. Lilly does not ship directly to patients; the treating physician must oversee how the medicine is received, stored and dispensed.

Could a retatrutide trial give me a placebo?

Yes. The recruiting Lilly record found in our October 16 search, SYNERGY-Outcomes, includes a retatrutide-versus-placebo substudy. Ask whether that substudy is screening locally and which groups you could be placed in before you sign anything.

Should I start an approved medicine now or wait for retatrutide?

That's a decision to make with your clinician. Retatrutide has no announced approval date, and the recruiting Lilly study found in our October 16 search is for a specific liver condition. Don't stop or delay a medicine you've been prescribed just to wait for an unconfirmed trial spot or a launch.

The right GLP-1 provider isn't the same for everyone. It depends on whether you'll use insurance or pay cash, whether you want an FDA-approved or compounded medication, your preferred route (injection or oral), and your budget. Answer a few questions in The RX Index's Find My GLP-1 Path tool to see a suggested provider route and pricing to review before you choose. It does not check your plan's coverage or copay; confirm those in your insurer's member portal or with the pharmacy-benefits number on your card.

  • What it asks: your goals, how you plan to pay, and your treatment preferences
  • What you get: a suggested provider route and pricing information to review
