RETATRUTIDE HALF-LIFE

Retatrutide Half-Life: About 6 Days, and How Long It Stays in Your System

By The RX Index Editorial Team · Last verified

The retatrutide half-life is about 6 days in Lilly's early human studies. That means the amount in your body drops by about half every 6 days, not all at once. A model leaves about 3% after 30 days of decline, yet a small 2026 blood study still found it 49 days after a second shot. Retatrutide is not FDA-approved.

Six days, 30 days, and 49 days are all true. They just measure different things. Here's how to tell them apart — and which of these numbers you should never use to plan a dose or a switch.

Four numbers people mix up

Retatrutide's half-life tells you how fast the level in your blood falls. It doesn't tell you when the drug is gone, when a lab test would come back negative, or when you'll stop feeling it. Those are four different clocks.

The short version: a half-life is a speed. A positive blood test is a test result. How long you feel something is a medical question. They aren't the same clock.

How much retatrutide is left after 1, 2, 4, or 6 weeks?

With a 6-day half-life and no more shots, about 45% is left after 1 week, about 20% after 2 weeks, about 4% after 4 weeks, and under 1% after 6 weeks. That's simple math from Lilly's early studies, not a measurement of anyone's blood.

How we figured it: amount left = 100% × ½ raised to (days ÷ half-life). The 5-to-7-day range is how Lilly's own trial paperwork describes the half-life across dose groups (NCT04881760 protocol). The model starts with a reference blood level that is already falling — not the moment of the shot. In Lilly's studies, levels usually peaked about half a day to 3 days after a shot. The math assumes no more doses. Real bodies vary, and this doesn't apply to a product whose contents nobody has checked.

Picture 100 marbles in a jar. Every 6 days, you take half of them out. After 6 days there are 50. After 12 days, 25. After 30 days, about 3 marbles are still rattling around. The jar isn't empty. It's just close — and a sensitive enough scale could still tell.

Want this laid out on a calendar? Put in the date of a last shot, and the timeline below puts the half-life math on dates you can see. It's a model, not a reading of how much is in your blood on those dates. Levels can rise after a shot, and older doses can still be in your system. It isn't a lab result, a switch date, or medical advice.

The center uses a 6-day half-life; the range uses 5 to 7 days. These are mapped intervals of an assumed falling reference amount, not pharmacokinetic predictions measured after shots.

In a separate 2026 study of 4 people who self-administered two shots 7 days apart, a sensitive blood test still found retatrutide 49 days after their second shot. This cannot predict your test or a safe switch date. Read the 2026 study.

This is an example of half-life math, NOT your blood level or amount left after an injection: levels rise before they fall, and earlier shots can overlap. Not a blood test, a safe-switch or surgery date, or medical advice. This cannot tell you what is inside an online vial.

We don't store or send anything you enter.

What changes with weekly shots?

With a shot every week, each new dose lands on top of what's left from the last one. Levels climb for roughly the first 4 weeks, then level off. That's half-life math (4 to 5 half-lives), and Lilly's 2022 study said the 6-day half-life lets weekly shots reach and hold steady levels (Urva et al., The Lancet).

It also means a table that starts from one shot can't tell you anyone's real level after months of shots. For the dose steps Lilly actually used in its trials, see retatrutide dosing studied in trials.

Retatrutide half-life vs Zepbound, Wegovy, and Foundayo

Retatrutide's half-life is about 6 days. Tirzepatide (Zepbound) is about 5 to 6 days and semaglutide (Wegovy) is about 1 week, so the three weekly shots fade at similar speeds. Foundayo, a daily pill, clears in days. The bigger difference is that the other three are FDA-approved and come with official rules.

The "left after" numbers are simple falling-level models, not a measurement after a last dose. Zepbound's label lists about 5–6 days for adults with overweight or obesity. Close half-lives don't make drugs equally safe, equally effective, or swappable dose for dose. For the full head-to-heads, see retatrutide vs tirzepatide, retatrutide vs semaglutide, and retatrutide vs Foundayo.

Here's the part that matters if you'd rather start something now than wait. Zepbound, Wegovy, and Foundayo have FDA labels, set dose steps, and missed-dose rules. Retatrutide has none of those yet, and it can't be compounded. Which approved option fits you isn't the same for everyone — it depends on your state, your insurance and formulary (your plan's list of covered drugs), whether you want an FDA-approved option or have questions about a separately regulated compounded medicine, whether a shot or a pill suits you, and your budget. Where does the 6-day number come from?

Two small Lilly studies from 2022 measured it. A single-shot study in healthy adults and a 12-week study in adults with type 2 diabetes both found a half-life of about 6 days. The big Phase 3 trials haven't published a half-life in the sources we checked.

That's 117 people enrolled across two short studies, including the placebo and comparison groups — not 117 people who all got retatrutide. The half-life is well supported for what these studies measured. We didn't find an updated half-life from the larger Phase 3 trials.

Why do some sites say 6 to 8 days?

We found no Lilly source for 8 days. Lilly's own trial paperwork describes about 5 to 7 days across dose groups, and its papers round that to about 6. If a page tells you 6–8, check what it cites.

Why does it last so long?

Retatrutide is a small protein (39 amino acids) with a fatty-acid tail attached (Urva et al., 2022). Tirzepatide carries the same kind of tail, and its FDA label says that tail lets the drug ride on albumin, a protein in your blood, which slows how fast it's cleared (Zepbound label). That's why both are weekly shots instead of daily ones.

Does a bigger dose last longer?

The speed it leaves stayed about the same across the doses studied. In the 12-week study, blood levels rose in step with the dose, and the half-life stayed about 6 days (Urva et al., 2022). A bigger dose starts from a higher level, so it takes a little longer to fall to the same low point.

When does retatrutide peak?

After a shot, retatrutide reached its highest level about 12 to 48 hours later in the type 2 diabetes study, and about 1 to 3 days later in the healthy-adult study. Then it fell slowly through the week.

The peak isn't when it "works best." In the trials, weight and blood sugar changes built over weeks, not hours (Urva et al., 2022).

Is retatrutide gone after 30 days?

Not necessarily. Thirty days is five half-lives, which leaves about 3% in the simple math. A small 2026 lab study still found retatrutide in the blood of all four volunteers 49 days after their second shot. No study has set a day when it's fully gone for everyone.

Keep its limits in view. It was four people who each self-administered two 2-mg doses, a week apart. Testing stopped at day 56 after the first dose, so the study couldn't tell when those blood tests would have turned negative. It measured whether a lab could find the drug — not whether it still worked or still caused side effects. And it was separate from Lilly's drug trials.

How can 6 days and 49 days both be right?

Think of a dimmer switch that cuts the light in half every 6 days. After a month the room looks dark. But a very sensitive light meter can still pick up a faint glow weeks later. The half-life is the dimmer. The 49-day result is the light meter. A faint signal doesn't mean the lamp is still lighting the room.

Does half-life tell you when effects or side effects stop?

No. How much drug is in your blood, how strongly you feel it, and how long a side effect lasts are three different things. In Lilly's early studies, some effects faded within days and others lasted weeks.

  • Heart rate and blood pressure: After one shot in healthy adults, heart rate went up and blood pressure went down at higher doses. Both were back near normal by day 29 (Lilly study protocol).
  • Weight: After a single shot, weight stayed lower through day 43 (Coskun et al., 2022).
  • Stomach side effects: In the 12-week diabetes study, most were mild or moderate and faded within about 10 days of when each symptom began, even while people kept taking it (Urva et al., 2022).

These come from small, early studies. They describe groups, not what you'll feel. For the full trial picture, see retatrutide side effects reported in trials and retatrutide and heart rate.

Here's how to match your real question to what the evidence can — and can't — answer:

Does half-life tell you when effects or side effects stop?
Your real questionWhat the evidence can answerWhat to do if it affects your health
"Is it gone 6 days after my last shot?"No. A falling blood level takes about 6 days to halve; that is not the measured amount after a shot.Nothing, if you're just curious.
"Is it completely gone at 30 days?"No set "fully gone" day exists.Ask the clinician who manages your care.
"When will my side effects stop?"Half-life can't predict this.Ask a clinician. Get urgent help for severe symptoms.
"Can I switch medicines after a certain number of days?"No tested retatrutide switch timing or dose conversion exists.Ask the prescriber who'll manage the new medicine.
"Would a lab test find it?"In 4 people, yes, 49 days after a second shot.Tests differ. This can't predict your result.
"Is a vial sold online the real thing?"No website can tell you what's inside.Don't rely on it. The FDA says these products are of unknown quality.

If you used a product sold as retatrutide and feel unwell, don't wait on a half-life chart to decide whether to get help. In the U.S., call Poison Control at 1-800-222-1222 (free, 24/7), or 911 for trouble breathing, a seizure, or someone who can't be woken up.

Can you take retatrutide twice a week?

The published Lilly repeat-dose trials we checked gave retatrutide once a week. We found no controlled evidence that twice-weekly, every-other-day, or "microdose" schedules work the same or are as safe. Half-life math can draw a smoother line on a chart. It can't prove safety.

We know many people asking this are holding a vial they bought online. We're not going to give you a split schedule, because no number on any website can tell you what's in that vial or how strong it is. The FDA has warned companies that sold retatrutide labeled "for research purposes" or "not for human consumption" with dosing instructions, and it urges people not to buy these products because their quality is unknown (FDA).

What actually helps is telling a clinician the truth. You can use this sentence word for word:

"I've been using a product sold as retatrutide. I don't know exactly what's in it. I started around [date], last took about [amount] on [date], and here's what I've noticed."

If you don't know an amount, say so. For more, see whether compounded retatrutide is legal and how to avoid fake GLP-1 medications online.

What if a retatrutide dose is missed?

There's no FDA-approved missed-dose rule for retatrutide. People in a trial should call their study team. Don't borrow Zepbound's or Wegovy's rule — those were written for those drugs.

Because the half-life is long, levels fall gradually instead of dropping to zero the day after a missed shot. Approved weekly drugs have product-specific missed-dose rules. Retatrutide has no approved missed-dose window. For the approved drugs' rules, see Wegovy and Zepbound missed-dose rules.

Can you use the half-life to plan stopping, switching, or surgery?

No. The half-life tells you roughly how long a drug lingers, not when it's safe to start another one. There are no approved switching rules for retatrutide, so a clinician or study team makes those calls.

Bring your last-dose date to that conversation. The timeline tool above turns it into a calendar.

Is retatrutide FDA-approved or available?

No. As of October 26, 2026, retatrutide isn't FDA-approved, and Lilly says it plans to file for approval in the first quarter of 2027. The only medical ways to get it are a clinical trial or Lilly's narrow, doctor-requested expanded-access program.

  • Filing plan: Lilly's second-quarter 2026 results said its clinical data package is complete and it plans to submit a Biologics License Application (its request for FDA approval) in the first quarter of 2027 (Lilly, Aug 5, 2026). A filing isn't an approval.
  • Compounding: The FDA says retatrutide can't be used in compounding under federal law (FDA, content current as of Oct 1, 2026).
  • Expanded access: Lilly lists a pre-approval program that a treating doctor must request, with strict criteria (ClinicalTrials.gov NCT07629401). Meeting the criteria doesn't guarantee access.

For the full picture, see how to get retatrutide and who qualifies for retatrutide early access.

What we actually verified

We checked the published human studies, Lilly's trial paperwork, a 2026 lab detection study, current FDA statements, and the FDA labels for the approved drugs we compare. We didn't test any product, follow any patients, or work out a safe stopping or switching schedule.

Checked October 26, 2026:

  • Lilly Medical's retatrutide answer page (half-life about 6 days; Phase 1; healthy adults with BMI 19–40; page last reviewed March 18, 2024)
  • Urva et al., The Lancet, 2022, original paper (half-life about 6 days; peak 12–48 hours; dose-proportional levels; most stomach side effects resolved within about 10 days after onset)
  • Coskun et al., Cell Metabolism, 2022, original publication and indexed Table 1 (doses 0.1–6 mg; weight lower through day 43; group geometric mean half-lives 134–165 hours)
  • Lilly's Phase 2 protocol on ClinicalTrials.gov, NCT04881760 (45 healthy adults; peak 1–3 days; half-life about 5–7 days across doses; heart rate and blood pressure near normal by day 29)
  • Carneiro et al., Rapid Communications in Mass Spectrometry, 2026, original open-access full text and Table 3 (4 volunteers given two doses, plasma detection at day 49 after their second dose; 7.1–38.4 ng/mL)

  • FDA's page on unapproved GLP-1 drugs (content current as of Oct 1, 2026); Lilly's Aug 5, 2026 results; ClinicalTrials.gov NCT07629401

Our own math: every "left after" percentage, the 5-to-7-day ranges, the about-4-week build-up, and the timeline dates. Example: 30 days ÷ 6 days = 5 half-lives, and ½ × ½ × ½ × ½ × ½ = 3.1%.

What we couldn't confirm: an updated half-life from the large Phase 3 trials or published results from Lilly's kidney and liver studies of retatrutide (NCT05611957 and NCT05916560). The relevant registry results pages weren't fully accessible during this check, so we aren't claiming no results exist. We reviewed the original published pharmacokinetic table and the full 2026 blood-detection paper.

What we didn't do: we didn't buy, test, or use any product. This page has no medical reviewer.

Retatrutide half-life FAQ

How long does retatrutide stay in your body?

In a simple falling-level model, about 3% of the starting amount remains after 30 days. But a small 2026 study still detected it in four people 49 days after a second shot, and no study has set a "fully gone" day for everyone. Those numbers come from Lilly's study product, not products sold online.

When does retatrutide peak in your system?

About 12 to 48 hours after a shot in Lilly's type 2 diabetes study, and about 1 to 3 days after a shot in its healthy-adult study. After that, levels fall slowly through the week.

Can you take retatrutide twice a week?

Not in any published trial. Lilly's repeat-dose trials used one shot a week, and twice-weekly or split dosing hasn't been studied for safety or effect. If you're in a trial, ask your study team; otherwise, ask a clinician.

Which GLP-1 has the shortest half-life?

Short-acting exenatide (the drug in Byetta and its generic versions) has a half-life of about 2.4 hours, according to its U.S. label; brand-name Byetta was discontinued, so check current availability. Liraglutide (Saxenda, Victoza) lasts about 13 hours, and the daily Foundayo pill about 29 to 49 hours. Among the drugs in our comparison table, Foundayo is shortest. Retatrutide, at about 6 days, is in the long-acting group.

When do you stop feeling retatrutide's effects after stopping?

The half-life can't tell you exactly. After one shot in Lilly's early study, heart rate and blood pressure were back near normal by day 29, while weight stayed lower through day 43. Your own experience may differ, so ask a clinician about symptoms that worry you.

What is retatrutide's half-life in hours?

About 6 days is roughly 144 hours. In Lilly's single-shot study, group geometric mean half-lives ran from 134 to 165 hours, so 144 is a handy round number, not one exact value for everyone.

Is retatrutide's half-life longer than tirzepatide's?

Slightly, or about the same. Retatrutide is about 6 days in early studies, and Zepbound's label lists about 5–6 days for adults with overweight or obesity. A similar half-life doesn't make them interchangeable.

Is retatrutide completely gone after 30 days?

Not by the math. Thirty days is five half-lives, which leaves about 3% of the starting level, and a sensitive test still found it at day 49 in a small study. Gone "for practical purposes" and gone "to a lab" aren't the same.

Does the 6-day half-life apply to retatrutide bought online?

Nobody can say. The number describes Lilly's study product, given in trials. A vial with unchecked contents or strength has unknown math, and the FDA says these products are of unknown quality.

Is retatrutide FDA-approved?

No. Retatrutide is investigational as of October 26, 2026. Lilly plans to file for FDA approval in the first quarter of 2027, and the FDA says it can't be used in compounding under federal law.

Find My GLP-1 Path

The right GLP-1 provider isn't the same for everyone. It depends on whether you'll use insurance or pay cash, whether you want an FDA-approved or compounded medication, your preferred route (injection or oral), and your budget. Answer a few questions in The RX Index's Find My GLP-1 Path tool to see a suggested provider route and pricing to review before you choose. It does not check your plan's coverage or copay; confirm those in your insurer's member portal or with the pharmacy-benefits number on your card.

  • What it asks: your goals, how you plan to pay, and your treatment preferences
  • What you get: a suggested provider route and pricing information to review
