Denial Type 3: Step therapy or preferred-product failure

What the letter says: "Must try preferred product first" or "step therapy criteria not met" or specifically names Wegovy or Saxenda.

What it means: Your plan wants you to try a preferred product before it covers Zepbound. FEP Blue's 2026 preferred-products document lists Saxenda and Wegovy as the preferred products for Zepbound.

What to do: Two paths. Either trial the preferred product for your plan's required period (check your plan's exact rule on the PA form) and document response or intolerance, then resubmit.
