Yes, if you have type 2 diabetes. Every Ambetter drug list we checked, across 28 states, puts Ozempic pens and tablets on Tier 2 (preferred brand) with prior authorization and a limit of about one pen per 28 days. Ozempic for weight loss alone isn't covered: Ambetter's GLP-1 policy requires a type 2 diabetes diagnosis.
The RX Index is the independent GLP-1 decision resource that scores telehealth providers and treatment paths on clinical legitimacy, care quality, transparency, access, and cost, so readers can choose the path that fits their situation.
Ambetter is Centene's Marketplace (ACA exchange) plan, and each state has its own drug list. On November 4, 2026 we opened all of them: 27 standard 2026 lists, the 25 standard 2027 lists posted so far, and California's two Ambetter from Health Net lists. Below: state results, what prior authorization asks for, cost, denials and the cash price.

| Tier | Tier 2, the preferred-brand tier (a medium copay on standard Ambetter lists) |
| Prior authorization | Yes. Listed as "Smart PA" on standard lists; "PA" on California's 2026 list and "Smart PA" on its 2027 list |

Is Ozempic covered by Ambetter in my state?
Almost certainly, if your state is one of the 28 below and you have type 2 diabetes. We found no state where Ozempic was missing, non-formulary or on a higher tier. On all 52 standard lists (2026 and 2027), every Ozempic entry reads the same:
| Ozempic product on the list | Tier | Requirements, word for word |
|---|---|---|
| Ozempic pen, 0.25 or 0.5 mg dose | 2 | Smart PA; QL(0.108 ML daily) |
| Ozempic pen, 1 mg dose (4 mg/3 mL) | 2 | Smart PA; QL(0.108 ML daily) |
| Ozempic pen, 2 mg dose | 2 | Smart PA; QL(0.108 ML daily) |
| Ozempic tablets 1.5 mg, 4 mg, 9 mg | 2 | Smart PA; QL(1 EA daily) |
California runs differently. Ambetter there is Ambetter from Health Net, with its own drug list. Ozempic pens are still Tier 2 ("Preferred Brand/High Cost Generics"), with "PA" on the list updated November 1, 2026 and "Smart PA" for 2027. The limit is 3 mL per 28 days at retail (30 days by mail), and tablets are capped at 30 per 30 days.

What does Ambetter's prior authorization for Ozempic require?
Ambetter plans follow Centene's Marketplace policy for GLP-1 drugs, HIM.PA.53 (last reviewed September 2026). It names Ozempic as a "preferred" GLP-1. For preferred drugs, your prescriber has to show all four of these:
- You have a diagnosis of type 2 diabetes.
- The request is for one of the preferred drugs: liraglutide (generic Victoza), Mounjaro, Trulicity, Ozempic, Rybelsus, Soliqua or Xultophy.
- You aren't taking another GLP-1 at the same time.
- The dose isn't above the FDA maximum. For Ozempic shots that's 2 mg a week. For Ozempic tablets it's 9 mg a day.
Approval lasts 12 months. To renew, your prescriber shows you're responding well to Ozempic, and the renewal also lasts 12 months.
There's no metformin-first step for Ozempic; the step rules apply only to brand Victoza, Bydureon BCise and Byetta. The policy lists no BMI or A1C cutoff for Ozempic, but your prescriber must send records "such as office chart notes, lab results or other clinical information," and Ambetter's request form asks for labs such as hemoglobin A1C where relevant.
What "Smart PA" means. Ambetter's drug lists use the label but don't define it. Either way, Ozempic still needs prior authorization, so ask Ambetter member services or your prescriber's office how the request works on your plan.
Where the request goes. Prescribers can fax Ambetter's prior authorization request form to (800) 977-4170 or submit it through CoverMyMeds. The form says Ambetter will respond by fax or phone within 24 hours of getting all the information it needs, except on weekends and holidays. In California, Health Net's drug list says requests can go through CoverMyMeds, by phone at 1-800-548-5524 or by fax at 1-800-314-6223. If Health Net doesn't answer a complete request within 72 hours (24 hours for urgent cases), it's deemed approved.
For a step-by-step on paperwork that works with any insurer, see our Ozempic prior authorization guide.
Does Ambetter cover Ozempic for weight loss?
No. Ozempic isn't FDA-approved for weight loss. Its label covers adults with type 2 diabetes: blood sugar control, plus lowering heart and kidney risks in those with heart disease or chronic kidney disease.
Ambetter's GLP-1 policy follows the label: non-FDA-approved uses aren't authorized except under Centene's separate off-label policy, and every Ozempic approval path starts with a type 2 diabetes diagnosis. An Arkansas Ambetter member notice dated November 21, 2025 says "Ambetter does not currently cover GLP-1s when prescribed solely for weight management," and that if you take one for weight loss without coverage, "you may be responsible for the full cost out of pocket."
California is the one place we found weight-loss coverage. Health Net's Ambetter lists say "Most plans do cover weight loss medications after prior authorization has been approved," after six months in a diet, exercise and behavior program and meeting your plan's BMI rules. The weight-loss drugs it names include Wegovy, Zepbound and liraglutide (Saxenda), not Ozempic.
No standard Ambetter list we opened includes Wegovy, Zepbound or Saxenda. For weight-loss rules, see does Ambetter cover Wegovy and does Ambetter cover Zepbound, and our Ozempic vs Wegovy comparison explains the two semaglutide brands.
How much will Ozempic cost with Ambetter?
You pay your plan's Tier 2 cost-share, which Ambetter's lists describe as a medium copay. Your exact copay or coinsurance is in your plan's Summary of Benefits and Coverage. Three things move the number:
- Deductible. Ambetter's Texas list says that until you meet your deductible, you pay the full cost of the drug. Check whether your plan's deductible applies to drugs.
- Supply. The Texas list says you generally pay one copay per 30-day supply. The pen limit (0.108 mL a day) is 3.024 mL over 28 days by our math: one 3 mL pen, which holds 4 weekly doses at 0.5, 1 or 2 mg.
- The $25 savings offer. With Ozempic coverage, Novo Nordisk's offer lets you pay as little as $25, with savings capped at $100 a month, for up to 48 months from enrollment. Its terms say Affordable Care (Health Exchange) plans aren't government programs for this offer, so Ambetter members can qualify. One catch: if your plan runs an accumulator or maximizer program, the terms say not to use it. Ask Ambetter first.
Our Ozempic cost with insurance guide walks through copays, coinsurance and the savings card across plan types, and our Ozempic savings card page covers how to activate it.
How do I check my own Ambetter plan?
Five steps, in this order:
- Find your state's pharmacy page at ambetterhealth.com/en/[your state code]/resources/pharmacy-resources/, like the Texas pharmacy resources page. California uses Health Net's Ambetter drug list page.
- Open the formulary for your plan year (2026 now, 2027 from January 1). Search the PDF for "OZEMPIC" and note the tier, PA and QL.
- Check your Summary of Benefits and Coverage for your Tier 2 copay or coinsurance and whether your drug deductible applies.
- Call member services (the number is on your ID card) and ask: "Is Ozempic covered for type 2 diabetes on my plan, does it need prior authorization, and what will I pay at my pharmacy?"
- Ask your prescriber's office to start the prior authorization if needed, with your diagnosis and recent A1C ready.
Rather not make the calls? Ro's free GLP-1 coverage check phones your insurer and emails you a report on whether your plan covers the Ozempic pen and needs prior authorization. Ro says the check doesn't submit a treatment request or write a prescription.
What if Ambetter denies Ozempic?
Based on Ambetter's own criteria, an Ozempic request can be turned down if there's no type 2 diabetes diagnosis on file, records are missing, or the request goes past the quantity limit. Here's what to do:
- Read the denial notice. HealthCare.gov says your insurer must tell you in writing why it denied the request.
- Fix and resubmit if it was paperwork. If the diagnosis code or lab results weren't sent, your prescriber can resubmit with chart notes and a recent A1C.
- File an internal appeal. HealthCare.gov says you must file within 180 days (6 months) of the denial notice, and your state's Consumer Assistance Program can file it for you. Ambetter's Texas drug list says appeals there are resolved within 30 days.
- Ask for an external review within 4 months if the appeal fails. Your insurer must accept the reviewer's decision. In urgent cases, you can request it alongside your internal appeal.
- Ask about a preferred alternative. Mounjaro, Trulicity and generic liraglutide are on the same preferred list. Mounjaro and Trulicity are Tier 2 with Smart PA on the standard lists; liraglutide is Tier 1B (1A in New Jersey).
If you were denied because Ozempic was prescribed for weight loss without diabetes, an appeal is unlikely to help. Your realistic paths are a covered weight-loss drug (rare outside California) or cash pay.
Our how to appeal an Ozempic denial guide has a ready-to-copy appeal letter. If you'd like help with the paperwork, Ro says its insurance concierge submits prior authorizations for members and can file an appeal if coverage is denied. That comes with Ro's paid Ro Body membership ($39 the first month, then $74 to $149 a month), which insurance doesn't cover. Ro's program is built around weight loss, and its own Ozempic page says Ozempic isn't FDA-approved for weight loss.

What does Ozempic cost without insurance?
If Ambetter won't cover it, Novo Nordisk sells Ozempic directly through NovoCare at these self-pay prices, as of November 4, 2026:
| Ozempic product | Self-pay price per month |
|---|---|
| Pen, 0.25 mg or 0.5 mg, new patients | $199 for each of the first 2 months (offer runs through December 31, 2026) |
| Pen, 0.25 mg, 0.5 mg or 1 mg | $349 |
| Pen, 2 mg | $499 |
| Tablets 1.5 mg (new patients) | $149 |
| Tablets 4 mg | $199 |
| Tablets 9 mg | $299 |
| List price, before any discount | $1,027.51 per pen |
Novo Nordisk defines a month as 1 box of 1 Ozempic pen, or 1 bottle of 30 tablets. NovoCare's page says pricing will be updated after December 31, 2026. Self-pay purchases are outside your insurance, so they don't count toward your Ambetter deductible or out-of-pocket maximum.
What about Ro? Its Ozempic page says that without insurance you'd pay list price, which Ro gives as $900 to $1,100, plus membership, so NovoCare is cheaper for Ozempic itself. Ro's posted cash prices are for other GLP-1s, mostly for weight loss: from $149 a month for the Wegovy pill or Foundayo pill, $199 for the Wegovy pen and $299 for the Zepbound KwikPen, plus membership.

Safety basics before you start
Ozempic's label carries the FDA's boxed warning about thyroid C-cell tumors seen in rodents. Do not use if you or a family member has had medullary thyroid cancer (MTC), or if you have MEN 2.
- Pregnancy: the label says to stop Ozempic at least 2 months before a planned pregnancy. Tell your prescriber if you're pregnant or planning to be.
- Pancreatitis: the label warns about pancreatitis. Get help right away for severe stomach pain that won't go away.
- This page isn't medical advice. A licensed clinician decides whether Ozempic is right for you and at what dose.
What we actually verified
All checks were done on November 4, 2026.
- We opened 54 Ambetter drug lists (27 standard 2026, 25 standard 2027, and California's 2026 and 2027 lists) and confirmed every Ozempic entry's tier, PA and quantity limit. All matched.
- We searched every list for Wegovy, Zepbound and Saxenda. None of the 52 standard lists include them; only California's Health Net lists do.
- We read Centene's HIM.PA.53 policy, the Arkansas member notice and Ambetter's prior authorization form.
- We checked Ozempic's current labels and every price on the NovoCare, Ozempic.com and Novo Nordisk pricing pages.
- We read Ro's Ozempic, insurance, pricing and coverage-check pages. Ro's claims are its own; we didn't test them.
- We couldn't confirm: your plan's copay (it varies by plan), how Ambetter's "Smart PA" works behind the scenes, whether Health Net in California uses the same GLP-1 criteria as other states, or Ambetter's 2027 lists for Delaware and New Hampshire, which weren't posted yet.
The RX Index earns a commission from some links on this page; that doesn't change what we report.
Frequently asked questions
Does Ambetter cover Ozempic?
Yes, for type 2 diabetes. All 28 Ambetter state drug lists we checked on November 4, 2026 put Ozempic on Tier 2 with prior authorization and a quantity limit.
Does Ambetter cover Ozempic for weight loss?
No. Ozempic isn't FDA-approved for weight loss, and Ambetter's GLP-1 policy requires a type 2 diabetes diagnosis. Ambetter's Arkansas plan has told members it doesn't cover GLP-1s prescribed only for weight management.
Does Ambetter require step therapy for Ozempic?
No. Ambetter's policy, HIM.PA.53, lists Ozempic as a preferred GLP-1 with no metformin-first step. Step therapy applies only to brand Victoza, Bydureon BCise and Byetta.
How much is Ozempic with Ambetter?
Your plan's Tier 2 copay or coinsurance, after any drug deductible. If Ambetter covers your fill, Novo Nordisk's savings offer can bring it to as little as $25 (savings capped at $100 a month).
Can I use the Ozempic savings card with an Ambetter plan?
Usually, yes. Novo Nordisk's terms say Health Exchange plans aren't government programs for the offer. You still need Ambetter to cover the fill, and you can't use the offer if your plan runs an accumulator or maximizer program.
Is Ozempic covered by Ambetter in 2027?
Yes, on every 2027 list posted so far: Tier 2 with Smart PA. Delaware and New Hampshire hadn't posted 2027 lists when we checked.
What if my Ambetter plan denies Ozempic?
Read the reason, have your prescriber resubmit with your diagnosis and A1C, and appeal within 180 days. If that fails, request an external review within 4 months. NovoCare cash pay starts at $199 a month for new patients.
