Ozempic Cost at Costco: What You'll Really Pay in 2026
By The RX Index Editorial Team · Last verified October 2026
Ozempic cost at Costco starts at $199 a month for your first two fills of the 0.25 mg or 0.5 mg pen, if you're new to Novo Nordisk's offer and fill by December 31, 2026. After that, it's $349 for strengths up to 1 mg and $499 for 2 mg. You need a prescription, and it's a cash price.
The tables below show every strength, what a year costs, and when your insurance will beat Costco. We checked the prices on Costco's and Novo Nordisk's own pages on October 28, 2026.

Decision Verdict
Costco is a good place to fill Ozempic if you're paying cash, but its price isn't lower than anywhere else. It matches Novo Nordisk's own self-pay price. If your commercial insurance covers Ozempic, your copay with Novo's savings card will usually cost less.
- Best for: Adults with type 2 diabetes who have a prescription, no Ozempic coverage, and like filling at Costco. It also suits people whose insurance denied Ozempic and who would rather pay a fixed price than wait on an appeal.
- Not ideal for: Anyone whose commercial plan covers Ozempic, since the savings card can bring a fill down to as little as $25, within a $100 monthly limit. It's also a poor fit for anyone on Medicare or Medicaid who wants the purchase to count toward their plan, and for anyone looking for a weight-loss drug: Ozempic isn't FDA-approved for weight management.
- The factor that matters most: Whether you're billing insurance. Costco's price is cash only, and cash payments don't count toward your deductible or out-of-pocket limit.
How much does Ozempic cost at Costco without insurance?
Costco lists Ozempic at three monthly prices. One month is one box with one pen, which Novo Nordisk's terms treat as a 28-day fill.
| Ozempic strength | Costco monthly price | Who gets it | 13 fills (364 days) at this price |
|---|---|---|---|
| 0.25 mg or 0.5 mg pen, first two fills | $199 | New patients only, through December 31, 2026 | Not available all year (two fills max) |
| 0.25 mg or 0.5 mg pen, after the first two fills | $349 | Everyone paying cash | $4,537 |
| 1 mg pen | $349 | Everyone paying cash | $4,537 |
| 2 mg pen | $499 | Everyone paying cash | $6,487 |
Sources: Costco Member Value Pricing and Novo Nordisk's Ozempic self-pay terms, read October 28, 2026. The yearly totals are our math. Costco says shipping and handling aren't included in its prices.
Buying two or three months at once doesn't lower the price. Novo Nordisk's terms list a two-month fill of the 1 mg pen at $698 and a three-month fill at $1,047, exactly two and three times $349.
One oddity on Costco's page: the $349 (1 mg) and $499 (2 mg) cards are labeled "Ozempic Tablets." But 1 mg and 2 mg are pen strengths; the Ozempic pill comes in 1.5 mg, 4 mg and 9 mg. Ask the pharmacist which form your quote is for before you pay.

Who gets the $199 Ozempic price at Costco?
The $199 price matches Novo Nordisk's limited-time offer. Costco's page says it's limited to two fills, and Novo's terms spell out the rest. You qualify only if all of these are true:
- You haven't redeemed an Ozempic injection savings offer in the past 365 days. Novo calls this being a "new patient."
- Your prescription is for the 0.25 mg or 0.5 mg pen.
- It's one of your first two 28-day fills under the offer. Your third fill is $349.
- The offer is redeemed and the prescription is filled by December 31, 2026.
Costco's page says the price is $349 a month after December 31, 2026. Neither Costco nor Novo Nordisk has said what pricing will look like later in 2027, so don't count on the $199 price lasting.
Is Ozempic cheaper at Costco than anywhere else?
No. Costco's Ozempic prices are the same as Novo Nordisk's self-pay prices: $199 for eligible starter fills, then $349, or $499 for 2 mg. Novo offers those prices through NovoCare Pharmacy home delivery and, with a savings offer, at local pharmacies. The government's TrumpRx site lists the same prices on a coupon you show at a participating pharmacy.
| Where you fill | 0.25 mg or 0.5 mg, first two fills | 0.25 mg to 1 mg after that | 2 mg | Membership needed? |
|---|---|---|---|---|
| Costco Pharmacy | $199 | $349 | $499 | Costco calls it a member price |
| NovoCare Pharmacy (home delivery) | $199 | $349 | $499 | No |
| Local pharmacy with the TrumpRx or Novo self-pay coupon | $199 | $349 | $499 | No; the pharmacy must accept the offer |

So the reason to choose Costco is convenience, not a lower number. If you already shop there or like its pharmacists, it's a solid place to fill. If you don't, you can likely get the same price elsewhere without a membership.
Do you need a Costco membership to get Ozempic at Costco?
You don't need a membership to use a Costco pharmacy. Costco says you can buy prescriptions online or at its warehouses without being a member. But Costco calls this pricing Member Value Pricing and describes it as "a direct-to-members savings program."
We couldn't confirm whether a nonmember would be charged the same $199 or $349 at a Costco counter by showing Novo's or TrumpRx's self-pay coupon. Call your warehouse pharmacy and ask before you buy a membership just for this.
A basic Costco membership costs $65 a year, plus tax where it applies. Because NovoCare and the TrumpRx coupon charge the same as Costco, a membership bought only for Ozempic adds $65 without lowering the drug's price.
Does the Costco Executive membership pay for itself on Ozempic?
It can, slightly. Executive members pay an extra $65 a year and earn a 2% reward on most purchases. Costco says pharmacy purchases count "in most cases," using only what you pay out of pocket. Prescriptions don't count in some states, including Arkansas, New Jersey and New York.
| Your year of Ozempic (13 fills) | You pay | 2% reward | After the $65 upgrade fee |
|---|---|---|---|
| Two $199 fills, then 11 at $349 | $4,237 | $84.74 | About $20 ahead |
| All 13 at $349 | $4,537 | $90.74 | About $26 ahead |
| All 13 at $499 | $6,487 | $129.74 | About $65 ahead |
Our math, from Costco's Executive membership terms, read October 28, 2026. Sales tax on the fee is extra. Your strength over the year is your prescriber's call, so the rows show a range, not a plan.
The upgrade makes sense only if you already have a basic membership and live in a state where prescriptions count. Don't buy one expecting Ozempic alone to justify it.
How much is the Ozempic pill at Costco?
Costco's pricing page didn't list a price for the Ozempic pill in its actual strengths (1.5 mg, 4 mg and 9 mg) when we checked. Novo Nordisk's self-pay prices for the pill are:
| Ozempic pill | Self-pay price for 30 tablets | 12 bottles |
|---|---|---|
| 1.5 mg | $149 | $1,788 |
| 4 mg | $199 | $2,388 |
| 9 mg | $299 | $3,588 |

Novo says its pill self-pay pricing works at local pharmacies with a savings offer, so ask Costco whether it honors it. Whether the pill or the pen fits you is a question for your prescriber; the strengths aren't interchangeable. Our Ozempic pill reviews cover what's known so far.
Can you use insurance, Medicare or a coupon with Costco's Ozempic price?
Not together. Costco's price is cash only. Costco says claims sent to private insurance, Medicare, Medicaid or other public programs aren't eligible for its Member Value Pricing.
If you have commercial insurance that covers Ozempic: Use your coverage and Novo's savings card instead. Novo says you can pay as little as $25 for up to a 3-month prescription, with savings capped at $100 a month. That means a $90 monthly copay could drop to $25, but a $200 copay would still leave you paying $100. Either way, that's less than $349. See how the Ozempic savings card works and what Ozempic costs with insurance.
If your plan covers Ozempic but your share is more than $349: Paying cash at Costco could cost less that month. The catch is that cash payments don't count toward your deductible or out-of-pocket limit, so you'd lose that progress.
If you have Medicare: Novo's $25 savings card excludes government beneficiaries. You can still pay Novo's self-pay price, but you agree not to bill Medicare or count it toward your out-of-pocket costs. Check whether your Part D plan covers Ozempic first; see does Medicare cover Ozempic. On January 1, 2027, Medicare's negotiated price for semaglutide takes effect: $274 for a 30-day supply. That's what Medicare plans pay, not your copay. The Medicare GLP-1 Bridge doesn't include Ozempic, and people with type 2 diabetes aren't eligible for it.
If you have Medicaid: Check your state plan first; it may cover Ozempic for diabetes. See does Medicaid cover Ozempic.
HSA or FSA: Costco says it accepts HSA and FSA cards at its registers. Your plan decides what's eligible; see using an HSA or FSA for Ozempic.
Want Ozempic for weight loss? Check Wegovy's Costco price first
Ozempic is FDA-approved for adults with type 2 diabetes, to improve blood sugar and to lower certain heart and kidney risks. It isn't FDA-approved for weight management. Wegovy is Novo Nordisk's semaglutide medicine approved for weight management, and Costco prices it about the same:
- Wegovy pen: $199 for the first two 0.25 mg or 0.5 mg fills, then $349 at every strength Costco lists.
- Wegovy pill: $149 a month for 1.5 mg ($199 after December 31, 2026) and $299 for 9 mg or 25 mg, per Costco's page on October 28, 2026.
If weight loss is your goal, Wegovy is the semaglutide a prescriber would usually consider, and Costco's $499 price for 2 mg Ozempic is higher than any Wegovy pen price Costco lists. See how much Wegovy costs at Costco and Ozempic vs Wegovy.
How do you get an Ozempic prescription sent to Costco?
Costco needs a valid prescription. There are two common ways to get one there:
- Your own doctor. Ask them to send it electronically to your Costco pharmacy. If it already went to another pharmacy, you can ask Costco to move it.
- An online provider. Many telehealth services can prescribe Ozempic for type 2 diabetes. Before you pay, ask whether they'll send the prescription to the pharmacy you choose, since some use their own partner pharmacies. See how to get Ozempic online.
Ozempic from Costco is the same FDA-approved Novo Nordisk product you'd get at any other licensed pharmacy. It isn't compounded semaglutide.
What to say at the Costco pharmacy counter
Use these lines so nobody bills the wrong program:
- "I'm paying cash for Ozempic, not using insurance."
- "Is this the $199 new-patient price or the $349 price? I haven't used an Ozempic savings offer in the past year." (Only say the second part if it's true.)
- "Is this quote for the pen or the pill, and which strength?"
- "Do I need a Costco membership for this price, or can you apply Novo Nordisk's self-pay offer?"
- "Is it in stock today, and is there any shipping or handling charge?"
Which Ozempic route fits you
| Your situation | Cheapest likely route | What to check |
|---|---|---|
| Commercial plan covers Ozempic | Your copay plus Novo's savings card | Card terms and the $100 monthly limit |
| Plan denied Ozempic and you'll pay cash | Costco, NovoCare or a TrumpRx coupon: $199, then $349 | Whether the $199 starter rules fit you |
| Your prescription is the 2 mg pen and you pay cash | $499 at any of them | Whether an insurance appeal is worth filing |
| Medicare Part D | Your Part D plan | Formulary and copay; self-pay doesn't count toward your costs |
| You want weight loss, not diabetes care | Ask your prescriber about Wegovy | Wegovy at Costco |

Frequently asked questions
How much does each Ozempic cost?
Paying cash, each Ozempic pen box (one month) costs $349 at Costco for the 0.25 mg, 0.5 mg or 1 mg strength and $499 for 2 mg. New patients pay $199 for their first two 0.25 mg or 0.5 mg fills through December 31, 2026. The Ozempic pill is $149 to $299 a month through Novo Nordisk's self-pay offer.
Is Ozempic cheaper at Costco?
No. Costco charges the same as Novo Nordisk's self-pay prices, which you can also get through NovoCare Pharmacy or with the TrumpRx coupon at participating pharmacies. Costco is worth it for convenience, not a lower price. With commercial coverage, your copay and Novo's savings card will usually beat it.
Do I need a Costco membership to buy Ozempic at Costco?
You don't need a membership to fill prescriptions at Costco. Costco does describe its Ozempic pricing as a member program, and we couldn't confirm what a nonmember is charged. Ask your warehouse pharmacy, or get the same prices without a membership through NovoCare or TrumpRx.
Can I use insurance with Costco's Ozempic price?
No. Costco's price is cash only and can't be combined with private insurance, Medicare or Medicaid claims. If your plan covers Ozempic, use your coverage and Novo's savings card instead. Paying cash means the amount won't count toward your deductible.
Is the $199 Ozempic price at Costco real?
Yes, but only for new patients, only for the 0.25 mg or 0.5 mg pen, and only for your first two fills through December 31, 2026. New means you haven't used an Ozempic injection savings offer in the past 365 days. Your third fill is $349.
Is Ozempic from Costco real Ozempic?
Yes. Costco fills FDA-approved Ozempic made by Novo Nordisk, the same product any licensed pharmacy dispenses. It isn't compounded semaglutide, and you still need a valid prescription.
Can I get Ozempic at Costco for weight loss?
Ozempic is FDA-approved for type 2 diabetes, not weight management. If weight loss is your goal, ask a prescriber about Wegovy. Costco prices the Wegovy pen at $199 for eligible starter fills and $349 after that, with the Wegovy pill from $149.
