Does TRICARE cover Ozempic for type 2 diabetes?
Yes. TRICARE covers Ozempic for adults with type 2 diabetes when its clinical and approval rules are met. Express Scripts runs the standard TRICARE pharmacy benefit; US Family Health Plan uses separate pharmacy coverage. Your doctor needs the current form for your exact prescription. 1 17 28
A prior authorization (PA) is TRICARE's approval before it pays. Your doctor completes the paperwork for that review. The publicly available Express Scripts Ozempic form dated Nov. 9, 2022 asks about:
- Your type 2 diabetes diagnosis. No type 2 diagnosis means coverage is not approved on that form.
- Metformin. It asks whether you tried metformin and did not achieve blood sugar control, had kidney problems that prevent its use or lactic acidosis while taking it, or have a medical reason you must not take it.
- The lower-copay alternative. The prescriber must acknowledge the form's note about Trulicity. 4
Here's a useful detail from the actual paperwork. That dated form does not ask for a certain A1C number or a certain number of months on metformin. A1C is a blood test that shows your average blood sugar. Don't treat a website's "A1C over 7" or "90 days of metformin" rule as your current TRICARE requirement. Have your doctor check the live form. 4 5
Who can help with the paperwork: start with the doctor treating your diabetes. Before paying an online clinic, ask whether it will complete the required TRICARE forms. A clinic's cash-pay billing model alone does not answer that question. Ro specifically says it cannot coordinate GLP-1 coverage for government plans. 12

What Ozempic is FDA-approved for. The Ozempic injection is approved for three things in adults with type 2 diabetes:
- To lower blood sugar, along with diet and exercise.
- To lower the risk of heart attack, stroke, or heart-related death, in those with established heart disease.
- To lower the risk of worsening kidney function, kidney failure, and heart-related death, in those with chronic kidney disease. 7
It is not approved for weight loss. Its safety and effectiveness have not been established in children. 7
One honest warning: Don't ask a doctor to change your diagnosis to get coverage. Your chart should show your real conditions. If you have type 2 diabetes, ask about the coverage above. If you don't, keep reading for the weight-management options.
Does TRICARE cover Ozempic for weight loss?
No. TRICARE's Ozempic coverage is for type 2 diabetes, not weight loss alone. For weight management, TRICARE covers other drugs instead: Wegovy, Zepbound, and Saxenda, on eligible plans, with clinical and approval requirements. TRICARE For Life members don't get TRICARE drug coverage for weight loss. 1
You're not alone in asking. Dr. Paul Cordts, identified in TRICARE's August 2025 announcement as the Defense Health Agency's chief medical officer, said people were taking GLP-1s for weight loss "now more than ever." GLP-1s are the family of drugs Ozempic belongs to. But TRICARE draws a hard line between diabetes treatment and weight-loss coverage. 3
The covered option: Wegovy
Wegovy uses the same active ingredient as Ozempic, semaglutide, at doses FDA-approved for weight management. Both are brand-name, FDA-approved drugs. TRICARE lists Wegovy for weight management on these plans, when its clinical and approval rules are met: 1 30

TRICARE does not cover drugs for weight loss for TRICARE For Life, people with direct-care-only coverage, or certain other groups. Those members pay 100% for that excluded use, even with an approved form. A request for a different FDA-approved use needs its own coverage check; the weight-loss exclusion does not answer every possible use of Wegovy. 1 30

Our take: If you want semaglutide for weight loss, ask your doctor about Wegovy, not Ozempic. Here's why:
- It's the version TRICARE can approve for weight.
- At the listed standard cash prices, Wegovy through 2.4 mg costs the same as Ozempic through 1 mg, and less than Ozempic 2 mg.
- NovoCare, the drugmaker's pharmacy program, requires a prescription for an FDA-approved use. So cash Ozempic for weight loss alone is a dead end there too. 9

When the TRICARE path doesn't fit you
Start with the TRICARE Wegovy path before paying full cash price. But not everyone qualifies:
- It needs a network doctor.
- You must meet the clinical criteria on the applicable form.
- It needs a prior authorization review.
- TRICARE For Life and direct-care-only members can't use it for weight loss. 1
Ro does not take TRICARE or coordinate TRICARE GLP-1 coverage. Ro says on its own site that it can't coordinate GLP-1 coverage for government plans. 12

How much does Ozempic cost with TRICARE in 2026?
Under TRICARE's standard non-formulary copay schedule, an eligible Ozempic fill costs $85. Home delivery covers up to 90 days for that copay; a network retail fill covers up to 30 days. Approved medical necessity, protected beneficiary rates, and active-duty rules can lower the charge. 2 3
What "non-formulary" means. TRICARE's formulary is its list of preferred drugs. Non-formulary does not mean the drug is never covered. TRICARE identifies Trulicity as a lower-copay option and requires medical-necessity approval for Ozempic's lower formulary copay and military-pharmacy access. A PA is still required for Ozempic coverage. 1 3

A few fine-print notes:
- Non-network pharmacies work differently. You may pay up front, owe a deductible and cost-share, and need to file a claim. The $85 network table is not a non-network price promise. 2
- TRICARE's cost page shows two tables. One still lists older rates ($13, $38, $76). We used the table labeled for 2026 and 2027. 2
- Count days, not just pens. Ozempic's starter pen can provide six weekly doses under the labeled starter schedule. Other pen fills can cover four weeks. Use the days supplied on your actual prescription, not a blanket "13 pens a year" rule. 7
The $170 difference over the same 12 weeks
Same drug. Same $85 copay. Very different bill.
- Three eligible 28-day retail fills: $85 × 3 = $255.
- One eligible 84-day home-delivery fill: $85 × 1 = $85.
That's $170 less for the same 84 days in this example. It is not a guaranteed yearly saving or permission to refill early.
To switch, ask your doctor whether a 90-day prescription is appropriate and can be sent to TRICARE Pharmacy Home Delivery. An existing valid PA applies across TRICARE's military, home-delivery, and retail network pharmacies; changing the pharmacy alone does not require a new PA. You may still need to register for home delivery and confirm the allowed quantity. 5 27
Two reasons to check before switching:
- Other prescription insurance, including Medicare Part D: you generally cannot use TRICARE Home Delivery unless an exception applies, such as the other plan not covering the drug. 22
- Maintenance-drug rules: some medicines must move to home delivery or a military pharmacy after two retail fills. Ask whether your exact prescription has that rule before planning repeated $85 retail fills. 6
Can you get the lower $44 copay?
Maybe, but don't count on it. The lower copay needs a second approval called medical necessity (MN). It's your doctor's written reason you need a non-preferred drug. 3 5
Ask your doctor to get the current Ozempic MN form. Bring any records of Trulicity use, side effects, or reasons it may not fit your care. Having side effects is not the same as having an approved MN request. The reviewer must accept the documented reason under the applicable criteria.
If MN is approved under the standard schedule:
- You pay $44 by home delivery or $48 at a retail network pharmacy.
- A military pharmacy can fill it for $0, if it can supply the prescription. 2 3
Without that approval, compare the $85 home-delivery option before paying a higher bill elsewhere.
Active duty is different. Active duty members need approved medical necessity for a non-formulary drug. Once the required approvals are in place, covered military, network, and home-delivery fills cost $0. 2
Should you ask about Trulicity instead?
TRICARE identifies Trulicity (dulaglutide) as a lower-copay GLP-1 for diabetes. Two more points from TRICARE's published materials:
- The dated Ozempic PA form includes a note about Trulicity's cardiovascular risk-reduction use.
- TRICARE's FAQ includes a Trulicity PA exception when another diabetes drug appears in your TRICARE pharmacy record in the last 720 days. 1 4
Do not turn that 720-day note into an automatic Ozempic waiver. Which drug fits you is a medical call for you and your doctor.
TRICARE vs. paying cash
In the 84-day, 1 mg maintenance example above, covered home delivery is $85, compared with $1,047 at NovoCare's standard cash price. That is a $962 difference before any clinician fees. If you have type 2 diabetes, use your TRICARE benefit before paying cash when the prescription is covered. 2 9

Type 2 diabetes and metformin
The dated form starts with the type 2 diabetes diagnosis. It then asks whether metformin failed to control blood sugar, whether kidney problems that prevent its use or lactic acidosis occurred while taking it, or whether metformin is contraindicated—a medical reason not to use it. 4
Bring the history, not a guess. Your doctor needs the medicine name, dates, what happened, and why it was stopped or ruled out. Do not start, stop, or retry metformin to meet a rule copied from an article.
What if you also have chronic kidney disease?
Ozempic injection has an FDA-approved use to lower the risk of worsening kidney disease, kidney failure, and cardiovascular death in adults with both type 2 diabetes and chronic kidney disease. That is a drug-label fact, not an automatic TRICARE approval. 7
Bring your latest kidney labs and med list. Ask your doctor: "Which current Ozempic criteria apply to my diabetes and kidney disease, and what records do you need?"
The dated Express Scripts form linked here does not establish a separate kidney-approval path. Your doctor should use the current requirements from your pharmacy benefit, not assume that the FDA-approved kidney use removes the need for PA.

Who sends it, how long it takes, and whether it expires
- Your prescriber completes and signs it. The dated form allows provider phone or fax submission and also lets the patient mail the completed form with the prescription. Follow the current form's instructions. 4
- The traditional review takes about 10 days after Express Scripts receives the request, according to TRICARE. Electronic requests may move differently. Ask the office to confirm when it was received; this is not a promised approval date. 15
- One valid PA works across the standard program's pharmacy options: military pharmacies, home delivery, and retail network pharmacies. Stock, MN, and dispensing rules still apply. 5

Does your TRICARE plan change the answer?
For type 2 diabetes, Ozempic coverage remains available under TRICARE when the clinical and approval rules are met. Your beneficiary category changes the copay, and USFHP changes the pharmacy process. TRICARE For Life
Diabetes: the coverage path remains. The August 2025 change ended drug coverage for weight loss for affected beneficiaries, including TRICARE For Life. It did not end diabetes coverage for drugs like Ozempic. You also don't need to buy Medicare Part D to get drug coverage. The TRICARE pharmacy benefit already covers you. 3 16
Weight loss: look at the Medicare GLP-1 Bridge. From July 1, 2026, through Dec. 31, 2027, this Medicare program offers eligible beneficiaries certain drugs for $50 per monthly supply:
- Wegovy (shot or pill)
- Zepbound KwikPen, not its single-dose pen or vial
- The Foundayo pill
Ozempic isn't part of it. The Bridge requires enrollment in an eligible Part D plan, but runs outside the usual Part D coverage and payment process. TFL alone does not qualify you. 18 19
The health criteria matter too. You must be an adult using the medicine for weight management with ongoing diet and activity changes. At the start of GLP-1 treatment, the clinical criteria include one of these:

CMS criteria checked Sept. 23, 2026. BMI is a height-and-weight measure. The prescriber must confirm the full criteria; this table does not approve enrollment. 19
People with type 2 diabetes, moderate-to-severe obstructive sleep apnea, or the specified form of MASH liver disease are directed to the normal Part D coverage path instead. Medicare's patient guide also excludes people whose Part D plan has already paid for a GLP-1 in 2026. A prescription for cardiovascular risk reduction belongs with the Part D plan, even when weight loss is also a goal. 19 20
So here's the real decision: is the Bridge worth paying a Part D premium? Add that premium to the $50 drug payment. Bridge payments do not count toward the Part D deductible or out-of-pocket cap, and Zepbound KwikPen needles are separate. Adding other drug insurance can also change access to TRICARE Home Delivery. 18 19 22
Medicare's Open Enrollment runs Oct. 15 to Dec. 7 each year, with changes normally taking effect Jan. 1. Before enrolling just for this program, confirm the plan type, your clinical eligibility, and the total cost. 21
How the Medicare GLP-1 Bridge works →
Active duty
- You pay $0 for covered military, network, and home-delivery fills once the required approvals are in place.
- You need both PA and medical-necessity approval for Ozempic's non-formulary coverage. 2
- Even if the form points toward a denial, the published PA instructions say the first request still goes to the TRICARE pharmacy contractor. A military treatment facility can handle later reconsideration. 4
- Ask your military provider about any duty, flight, or deployment requirements before you start.
Active duty families on TRICARE Prime Remote
- Since Feb. 28, 2026, eligible family members on Prime Remote in the U.S. pay $0 for covered drugs through home delivery and retail network pharmacies; covered military-pharmacy fills also cost $0.
- Non-formulary drugs need approved medical necessity under the waiver. The announcement says that without it, the drug is available only at retail at full cost. Don't assume an approved PA alone gives you $0 Ozempic. 6
US Family Health Plan
USFHP has its own pharmacy coverage. Ask your USFHP plan which Ozempic form and pharmacy to use. Don't assume the standard Express Scripts steps apply. 17
Does TRICARE cover the Ozempic pill?
The Ozempic pill (semaglutide tablets, 1.5 mg, 4 mg, and 9 mg) became available in the U.S. on May 4, 2026, for adults with type 2 diabetes. As of Sept. 23, 2026, we could not confirm how TRICARE covers it. Don't assume the shot's coverage rules carry over to the pill. 8 29
What we do know:
- It's for type 2 diabetes. It lowers blood sugar with diet and exercise. It also lowers the risk of heart attack, stroke, or cardiovascular death in adults with type 2 diabetes at high risk. It isn't approved for weight loss. The Wegovy pill is a separate weight-management product. 8 30
- It's a newer version of the Rybelsus pill. The doses don't match Rybelsus mg-for-mg, so don't swap one for the other on your own. 8
- Cash price at NovoCare: $149 for 30 tablets at 1.5 mg, $199 at 4 mg, or $299 at 9 mg, checked Sept. 23, 2026. The 1.5 mg dose is a starter dose, not the ongoing blood-sugar-control dose. 8 9
Your move: ask your doctor or pharmacist to look up Ozempic tablets, your strength, and the exact product code in the TRICARE Formulary Search Tool. Record the tier, required form, quantity limit, and price. A result for the pen does not answer those questions for the tablet.
What if TRICARE denies Ozempic?
Start by separating three problems: an uncovered use, missing paperwork, or an approved prescription with a high copay. A missing piece can be fixed and resent. A pharmacy coverage denial can be appealed in writing within 90 calendar days of the decision. 13
Before you pay cash or sign up for anything, figure out which problem you have.

How to appeal (TRICARE's rules)
For an Express Scripts pharmacy appeal:
- Put it in writing and sign it.
- Say exactly why you disagree.
- Include a copy of the decision.
- Add supporting records. If you're still waiting on some, send the appeal anyway and say more is coming, with an expected date.
- It must be postmarked or received within 90 calendar days of the decision date.
- Mail to: Express Scripts, Inc., P.O. Box 60903, Phoenix, AZ 85082-0903.
- TRICARE pharmacy help: 1-877-363-1303.
- If Express Scripts says no again, you can ask for a second-level appeal. The letter tells you how. 13 14
Don't send a pharmacy appeal to your regional contractor's medical appeals address. It's a different process. USFHP members should follow their plan's decision letter.
A simple appeal opening:
I am appealing the pharmacy decision dated ___. The denied request is ___. I disagree because ___. The attached records address the reason given in the decision by showing ___. Additional records will follow by ___, if needed.
Your clinician should supply the medical explanation. Do not add a diagnosis or treatment history that is not in your records.

Can TRICARE members pay cash for Ozempic or Wegovy?
Yes. TRICARE members can pay cash outside their insurance. NovoCare's pharmacy-program terms allow eligible government-insured patients to self-pay for an FDA-approved use. That is different from the commercial Ozempic savings card, which excludes TRICARE. Purchases under the self-pay offer cannot be sent to insurance for repayment. 9 10
Three rules to know before you pay cash:
- The $25 savings card is off-limits. It's for commercial insurance only. Government beneficiaries, including TRICARE members, are excluded. 10
- NovoCare self-pay has its own rules. Your prescription must be for an FDA-approved use, and you can't send the bill to your insurance afterward. 9
- Self-pay offer purchases don't count toward your insurance caps. The offer terms bar counting them toward a deductible or out-of-pocket limit. TRICARE calls that limit the catastrophic cap. That does not mean every prescription you pay for up front is excluded: ordinary TRICARE pharmacy claims have separate reimbursement rules. 9 23

Starter-offer dates: NovoCare's Ozempic and Wegovy injection terms require an eligible prescription to be written and received by Dec. 31, 2026, and shipped by Jan. 31, 2027. Ro advertises a Dec. 31 sign-up deadline for its listed starter offer. Read the terms for the path you use; filling out an interest form alone does not meet every requirement. 9 11

- Type 2 diabetes: check TRICARE first. Compare home delivery when you're eligible.
- Weight loss, on Prime or Select: try the Wegovy prior authorization first.
- Weight loss, on TRICARE For Life: check whether you qualify for the Medicare GLP-1 Bridge before paying for Part D.
- Weight loss, denied, or done waiting: compare cash-pay FDA-approved care, including Ro, against the cost of using your own clinician.

About compounded semaglutide: It is not FDA-approved. FDA does not review compounded drugs for safety, effectiveness, and quality before they are marketed. They are not FDA-approved generic Ozempic. The comparison above keeps them separate from approved brand-name options. 26
Is Ozempic safe, and who shouldn't take it?
Ozempic is FDA-approved for adults with type 2 diabetes. It carries a boxed warning because it caused thyroid tumors in rodent studies; it isn't known whether it does in people. You shouldn't use it if you or a family member has had medullary thyroid cancer or MEN 2. The most common side effects are nausea, vomiting, diarrhea, stomach pain, and constipation. 7
Coverage and safety are separate questions. An approval doesn't mean the drug is right for you. From the Ozempic prescribing information:
Don't use it if:
- You or a family member has had medullary thyroid cancer (MTC).
- You have multiple endocrine neoplasia type 2 (MEN 2), a rare inherited condition.
- You've had a serious allergic reaction to semaglutide or another ingredient in the product. 7
Serious side effects can include:
- Inflammation of the pancreas (pancreatitis)
- Gallbladder problems
- Vision changes
- Kidney problems from dehydration
- Severe stomach problems
- Serious allergic reactions
- Low blood sugar, when used with insulin or a sulfonylurea
- Food getting into the lungs during surgery or procedures with anesthesia 7
Tell your doctor if:
- You are pregnant or plan to be. The label says to stop Ozempic at least 2 months before a planned pregnancy; arrange the treatment plan with your doctor.
- You have had pancreas, kidney, stomach, or diabetes-related eye problems.
- You have a planned surgery or procedure. Tell the anesthesia team you take Ozempic. 7
Ozempic is not recommended for people with severe gastroparesis, a condition that greatly slows stomach emptying. 7
A verified regulatory issue: In a March 5, 2026 warning letter, FDA cited Novo Nordisk for failures in reporting and investigating adverse drug experiences, including semaglutide reports. FDA acknowledged corrective work but said the response lacked enough detail. The letter concerns safety-reporting failures; it is not an Ozempic recall or proof that the drug caused each reported event. 25

Does TRICARE cover Ozempic? Your other questions
The big rule is still type 2 diabetes plus the required approvals. The remaining questions concern your plan, forms, pharmacy, or product—not a promise that a prescription alone will be paid. 1
Is Ozempic covered by TRICARE Prime or TRICARE Select?
Yes, for type 2 diabetes when the coverage rules are met. The standard non-formulary copay is $85 per eligible fill. Home delivery can provide up to 90 days for that copay; retail network fills cover up to 30 days. 1 2
Does TRICARE For Life cover Ozempic?
Yes, for type 2 diabetes with the required approvals, through the TRICARE pharmacy benefit. TRICARE For Life doesn't cover drugs for weight loss. The Medicare GLP-1 Bridge may help with weight-management drugs other than Ozempic if you are enrolled in eligible Part D coverage and meet its rules. 1 18
Does TRICARE cover Ozempic for prediabetes, PCOS, insulin resistance, or type 1 diabetes?
Those diagnoses alone do not meet the published Ozempic type 2 diabetes coverage criteria. The public PA form reviewed here stops without a type 2 diabetes diagnosis. Ask your clinician about treatment and covered options for your actual condition. 4
How long do I have to take metformin before TRICARE covers Ozempic?
The public Nov. 9, 2022 Ozempic form sets no fixed duration. It asks about an unsuccessful metformin trial, specified problems during treatment, or a contraindication. Have your doctor check the current applicable form rather than relying on a universal number of days. 4 5
How long does TRICARE take to approve Ozempic?
TRICARE estimates about 10 days for the traditional PA process after Express Scripts receives the request. Ask your doctor's office to confirm the date they sent it, then check when Express Scripts received it. That estimate is not a guarantee of approval. 15
Does TRICARE's Ozempic approval expire?
Check your approval notice or ask Express Scripts for its start date, end date, and renewal rules. We did not verify a current rule that every Ozempic PA lasts forever.
Can I get Ozempic at a military pharmacy?
Medical-necessity approval is required for Ozempic's military-pharmacy coverage. Stock and dispensing rules also matter, so call ahead. PA alone does not guarantee that the pharmacy can fill it. 3
Can I use the Ozempic savings card with TRICARE?
No. Novo Nordisk's commercial-insurance savings card excludes government beneficiaries, including TRICARE members. That is separate from its self-pay offer. 10
Can I pay cash for Ozempic if I have TRICARE?
Yes. NovoCare's pharmacy terms allow eligible government-insured patients to self-pay for an FDA-approved use. You cannot claim repayment or count that offer purchase toward your insurance deductible or out-of-pocket cap. Ordinary pharmacy reimbursement claims are a separate process. 9 23
Can an online doctor get TRICARE to pay for my Ozempic?
An online prescription is not an insurance approval. Ask whether the clinician will complete the forms for your benefit before paying for a visit or membership. Ro says it cannot coordinate GLP-1 coverage for government plans, including TRICARE. 5 12
I have type 2 diabetes and obesity. Which rules apply?
The Ozempic diabetes path may apply when its clinical and approval requirements are met. Your doctor should record your real diagnoses on the form. Obesity does not erase a type 2 diabetes diagnosis or guarantee approval. 1
Is Wegovy the same as Ozempic?
Both contain semaglutide, but they're separate FDA-approved brands with different uses and doses. TRICARE lists Wegovy for weight management on eligible plans and Ozempic for type 2 diabetes. Don't swap the drugs or their doses yourself. 1 7 30
Does TRICARE cover compounded semaglutide?
Compounded semaglutide isn't FDA-approved. TRICARE's public GLP-1 page names brand-name drugs, but that list alone does not settle coverage of every compounded prescription. Ask your pharmacy administrator about the exact product before assuming it is covered. 1 26
Does TRICARE cover Ozempic overseas?
The diabetes coverage path remains, but overseas retail pharmacies use different cost-shares. TRICARE lists 20% after the deductible for active-duty families on Select Overseas and TRICARE Reserve Select; retirees and other Select Overseas beneficiaries generally pay 25% after the deductible. Active-duty families on Prime Overseas or Prime Remote Overseas have $0 covered pharmacy cost-shares. 2
Don't assume U.S. home-delivery arrangements work overseas. TRICARE lists country restrictions; confirm both the destination and the medicine's shipping requirements before relying on delivery. 17
