Trulicity vs Wegovy is a diabetes shot vs a weight-loss shot. Trulicity (dulaglutide) is a once-weekly injection from Eli Lilly FDA-approved for type 2 diabetes. Wegovy (semaglutide) is a once-weekly injection from Novo Nordisk FDA-approved for weight loss (and for lowering major heart events in adults who already have heart disease and obesity or overweight). They are not interchangeable, and Wegovy's label says not to combine it with any other GLP-1 medicine, Trulicity included.
So the real question is what you need treated: blood sugar, weight, or both — and what your plan will cover. Below: approvals, trial results, side effects and 2026 prices.

What is each one FDA-approved for?
Trulicity is approved for type 2 diabetes. Wegovy is approved for weight management (and related uses). That difference drives coverage, price and which one your prescriber is likely to suggest.
| FDA-approved use | Trulicity | Wegovy |
|---|---|---|
| Blood sugar control, adults with type 2 diabetes | Yes | No |
| Blood sugar control, kids 10 and older with type 2 diabetes | Yes | No |
| Lower risk of major heart events, adults with type 2 diabetes and heart disease or several heart risk factors | Yes | No (different population) |
| Weight loss and keeping it off | No | Yes |
| Lower risk of major heart events, adults with established heart disease and obesity or overweight | No | Yes |
| MASH with moderate to advanced liver fibrosis (adults) | No | Yes (accelerated approval) |
Semaglutide for diabetes is sold as Ozempic, not Wegovy. Tirzepatide for weight loss is sold as Zepbound — see Trulicity vs Zepbound and Zepbound vs Wegovy. The GLP-1 FDA indications chart maps every approved use.

Can you take Trulicity and Wegovy together?
No, not as a rule. Wegovy's label says using it with any other GLP-1 medicine isn't recommended, and Trulicity is a GLP-1 medicine. If you're on Trulicity and want to talk about Wegovy, that's a switch your prescriber plans, not an add-on.
Don't stop, start or change doses on your own. The two drugs aren't dosed milligram for milligram, and your prescriber decides where you'd start.
Which works better for weight loss?
Wegovy, by design — it is the one approved for weight loss. No trial has tested Wegovy against Trulicity for weight, so these numbers come from separate label studies:
| Trial (from each FDA label) | Who was in it | Dose | Weight change |
|---|---|---|---|
| Wegovy Study 2 (68 weeks) | Adults with obesity or overweight | 2.4 mg weekly | About 14.9% lost (placebo about 2.4%); difference about 12.4 percentage points |
| Wegovy Study 3 (68 weeks) | Adults with type 2 diabetes and BMI 27 or higher | 2.4 mg weekly | About 9.6% lost (placebo about 3.4%); difference about 6.2 percentage points |
| Trulicity AWARD-11 (36 weeks) | Adults with type 2 diabetes on metformin | 1.5 / 3 / 4.5 mg weekly | 3.0 / 3.8 / 4.6 kg lost (about 7 / 8 / 10 lb) |
What this means for you:
- Trulicity's weight loss is a side benefit, not its job. It has no weight-loss approval.
- Diabetes makes weight harder to lose on Wegovy too. Results were lower in Study 3 (with type 2 diabetes) than in Study 2.
- The trials differ in length and population, so treat this as a direction, not an exact race.
Heart outcomes: different people, different labels
Both drugs have FDA-approved heart-risk claims, but for different groups:
- Trulicity (REWIND): In adults with type 2 diabetes and heart disease or several heart risk factors, major heart events happened in 12.0% on Trulicity vs 13.4% on placebo over a median 5.4 years. The hazard ratio was 0.88.
- Wegovy (SELECT / Study 1): In 17,604 adults with established heart disease and obesity or overweight (people with diabetes were excluded), weekly Wegovy lowered the risk of major heart events vs placebo. The hazard ratio was 0.80 (95% CI 0.72 to 0.90). Median follow-up was about 41.8 months.
These are not head-to-head results against each other. Your diagnosis decides which heart claim, if any, applies.
Trulicity vs Wegovy side effects
They share the same main stomach side effects. Each label lists rates from its own trials, so these aren't head-to-head numbers:
| Nausea in each drug's own trials (not head to head) | Placebo | Drug |
|---|---|---|
| Trulicity 0.75 mg weekly (type 2 diabetes trials) | 5.3% | 12.4% |
| Trulicity 1.5 mg weekly (type 2 diabetes trials) | 5.3% | 21.1% |
| Wegovy 2.4 mg weekly (weight-loss trials) | 16% | 44% |
Wegovy's weight-loss trials also listed diarrhea (30% vs 16% placebo), vomiting (24% vs 6%), constipation (24% vs 11%) and belly pain (20% vs 10%).
Both labels carry the same boxed warning, the FDA's most serious kind: in rodents, both drugs caused thyroid C-cell tumors, and it isn't known if that happens in people. Do not use either if you or a family member has had medullary thyroid cancer (MTC), or if you have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Both labels also warn about pancreatitis, gallbladder problems, kidney injury from dehydration, and low blood sugar when used with insulin or sulfonylureas.

Trulicity vs Wegovy cost: which is cheaper?
Paying cash, Trulicity's flat LillyDirect price is often lower than Wegovy's ongoing $349 pen price — but Wegovy's limited-time intro and insurance rules change the math. With insurance, coverage usually follows the diagnosis on the prescription.
| Trulicity | Wegovy | |
|---|---|---|
| List / WAC | $1,006.93 a month (Lilly) | $1,349.02 per 4-pen package (Novo) |
| Cash / self-pay | Starting at $389 a month via LillyDirect (any dose) | $199/month for the first 2 months (0.25 or 0.5 mg), then $349; HD 7.2 mg $399 (intro terms through Dec 31, 2026) |
| TrumpRx (Nov 6, 2026) | Original $987.19; stated savings $598.19 (about $389) | Original $1,349.02; stated savings $1,150.02 (about $199) |
| Commercial insurance | Lilly says about 40% of patients pay $0 and 90% pay under $75 | As little as $25 a month with Novo's savings offer if your plan covers it (max savings $100/month; government beneficiaries excluded) |
TrumpRx's Trulicity "Original Price" ($987.19) is not the same number as Lilly's published list ($1,006.93); both are shown above and attributed. Plans cover Trulicity for type 2 diabetes much more often than they cover Wegovy for weight loss — check yours before you fill.

Which should you ask your doctor about?
Here's how the choice tends to break down. Your doctor decides what's right for you.
| If this sounds like you | What to ask about |
|---|---|
| You have type 2 diabetes and Trulicity controls it well | Staying on Trulicity, which is approved and usually covered for diabetes |
| You have type 2 diabetes and need more blood sugar or weight help | Other diabetes GLP-1s such as Mounjaro or Ozempic — see Trulicity vs Mounjaro and Trulicity vs Ozempic |
| You don't have diabetes and want weight loss | Wegovy or another weight-loss GLP-1, since Trulicity isn't approved for weight loss |
| You have obesity or overweight plus heart disease | Wegovy's SELECT heart indication may fit; Trulicity's heart claim is for people with type 2 diabetes |
| Your child, 10 or older, takes Trulicity for diabetes | Ask the pediatric team; Wegovy's teen approval is for obesity (12+), not for diabetes as this brand |
| You're paying cash | Compare LillyDirect $389 vs Wegovy's intro/$349 path by dose |

Coverage: why the diagnosis on the prescription matters
Insurance usually follows the FDA-labeled use. Plans that cover Trulicity for type 2 diabetes will not automatically cover Wegovy for weight loss, and the reverse is also true. Prior authorization, step therapy and BMI cutoffs are common on the weight-loss side.
A few practical patterns we see in companion RX Index guides:
- Diabetes diagnosis on the script tends to open Trulicity, Ozempic or Mounjaro pathways.
- Obesity or overweight-plus-condition on the script tends to open Wegovy or Zepbound pathways — with much more plan-to-plan variation.
- Cash-pay can bypass formulary fights, but you still need a valid prescription and you still pay membership or pharmacy fees where they apply.
Ro's free coverage check is built around brand-name pens it actually offers, including Wegovy. It is not a substitute for reading your own Summary of Benefits, but it can surface whether a prior authorization is likely before you fill.
How Trulicity and Wegovy compare with nearby options
People searching Trulicity vs Wegovy are often also weighing other weekly shots:
- Trulicity vs Ozempic: two diabetes GLP-1s (dulaglutide vs semaglutide 0.5–2 mg).
- Trulicity vs Mounjaro: dulaglutide vs tirzepatide for diabetes, including the SURPASS-CVOT heart trial.
- Trulicity vs Zepbound: Lilly diabetes shot vs Lilly weight-loss shot (same company, different labels).
- Zepbound vs Wegovy: the two leading weekly weight-loss brands.
- Ozempic vs Wegovy: same semaglutide molecule, diabetes brand vs weight brand.
If your clinician is choosing among diabetes options first, start with the diabetes matchups. If weight is the labeled goal and you do not have diabetes, Trulicity is not the approved tool for that job.
Practical next steps before you fill
- Confirm the labeled use that matches your diagnosis — diabetes, weight management, or heart-risk reduction in the population studied.
- Ask your plan (or Ro's coverage team for Wegovy) whether prior authorization is required and what BMI or A1C documentation they want.
- Compare cash paths only after you know the dose you would actually stay on. Wegovy's $199 intro is for the first two months at 0.25 or 0.5 mg only; maintenance is higher. Trulicity's LillyDirect price is flat across doses.
- Do not stop a diabetes medicine to chase weight loss without a replacement plan for blood sugar.
- Re-read the boxed warning: neither drug is for people with a personal or family history of MTC, or with MEN 2.

Frequently asked questions
Is Trulicity the same as Wegovy?
No. Trulicity is dulaglutide from Eli Lilly, approved for type 2 diabetes. Wegovy is semaglutide from Novo Nordisk, approved for weight loss (and related uses). Both are weekly GLP-1 shots, but they are different medicines with different labels.
Which is better for weight loss, Trulicity or Wegovy?
Wegovy. It is FDA-approved for weight loss; Trulicity is not. In separate label trials, Wegovy's average weight loss was much larger than the weight change seen with Trulicity in diabetes trials. No trial has compared them directly for weight.
Can I take Trulicity and Wegovy at the same time?
Wegovy's label says using it with any other GLP-1 medicine isn't recommended. Talk to your prescriber about switching instead of combining them.
Is Wegovy approved for type 2 diabetes?
Wegovy is not FDA-approved for blood sugar control in type 2 diabetes. Semaglutide for diabetes is sold as Ozempic. Wegovy is approved for weight management and for lowering major heart events in adults with established heart disease and obesity or overweight.
How much do Trulicity and Wegovy cost without insurance?
As of November 6, 2026, LillyDirect lists Trulicity starting at $389 a month. Novo's Wegovy self-pay path is $199 a month for the first two months at 0.25 or 0.5 mg, then $349 for most pen doses (HD 7.2 mg is $399), with intro terms through December 31, 2026. Your pharmacy quote can differ.
Does Ro offer Wegovy?
Yes. Ro lists Wegovy among the brand-name GLP-1s it offers, with a free coverage check and cash prices it says match NovoCare and TrumpRx. See our Ro Wegovy guide.
