Wegovy Cost With Insurance: What You'll Pay in 2026, Plan by Plan

By The RX Index Editorial Team
Last verified

Wegovy cost with insurance can be as little as $25 a month if your commercial plan covers it and you use Novo Nordisk's savings card, which takes off up to $100. Eligible people with Medicare pay $50 a month through the Medicare GLP-1 Bridge. Before you meet a deductible, you may owe your plan's full price for the fill.

The rest comes down to four things: whether your plan covers Wegovy at all, your copay or coinsurance, how much of your deductible is left, and whether your plan blocks the savings card. The table below shows what each type of coverage charges as of November 4, 2026.

The RX Index is the independent GLP-1 decision resource that scores telehealth providers and treatment paths on clinical legitimacy, care quality, transparency, access, and cost, so readers can choose the path that fits their situation.

How much is Wegovy with insurance?

It depends on your coverage type. Prices below come from Novo Nordisk's and Medicare's own pages as of November 4, 2026.

A "month" here means one box of four pens (28 days) or one bottle of 30 tablets, which is how Novo Nordisk defines it.

If you're still weighing whether insurance, cash or a telehealth provider is your best route, The RX Index's Find My GLP-1 Path tool sorts it out for you. It's free, takes about 2 minutes, and needs no signup.

If you don't know yet whether your plan covers the Wegovy pen, you can have someone call your insurer for you. Ro says its GLP-1 Insurance Coverage Checker checks coverage for the Wegovy pen for free. Ro calls your insurer to confirm eligibility only, without sending a prescription. The report tells you whether prior authorization is required and gives copay or cost estimates where they're available. Ro says the checker can't check the Wegovy pill.

What will I pay with a copay, coinsurance or deductible?

The savings card pays the difference between your plan's charge and $25, up to $100 a month. So once your plan charges more than $125 for a month, the card stops getting you to $25.

These are hypothetical examples for one month of the pen, not quotes. Your pharmacy's price is the one that counts.

What will I pay with a copay, coinsurance or deductible?
If your plan charges this for a monthCard takes offYou'd pay
A $40 copay$15$25
A $150 copay$100$50
20% coinsurance, if your plan's price were the $1,349.02 list price ($269.80)$100$169.80
The full price before your deductible, if that price were $1,349.02$100$1,249.02

Deductibles matter. In KFF's 2026 Employer Health Benefits Survey, the average single deductible was $1,955 for workers in plans with a general annual deductible. If your drug coverage falls under that deductible, your first fills of the year can cost far more than your later ones.

Your plan's negotiated price may be lower than the list price, so ask the pharmacy for the actual amount. If your insured price for a month comes out above the $349 self-pay price, cash is cheaper that month. But Novo Nordisk warns that money spent outside your insurance won't count toward your deductible or out-of-pocket limit. Our NovoCare cash vs insurance guide works through when to switch.

Your 5-question price check

Call your plan, or ask your pharmacist, and get answers to these:

  1. Is Wegovy on my formulary for weight management, and for which forms (pen, pill)?
  2. Does it need prior authorization or step therapy?
  3. What's my copay or coinsurance for a 28-day fill, and does the deductible apply first?
  4. How much of my deductible is left this year?
  5. Does my plan use an accumulator, copay maximizer or alternate funding program?

If you hear "covered, $40 copay, no accumulator," you're likely looking at $25 with the card. If you hear "excluded," skip ahead to the denial and cash sections.

How does the Wegovy savings card work with insurance?

The card is for people whose commercial insurance covers Wegovy. It's not a discount for people with no coverage. Here are the main rules from Novo Nordisk's terms as of November 4, 2026:

  • Coverage is required. You need commercial coverage for Wegovy and a prescription for an FDA-approved use.
  • There's a cap. The card covers up to $100 for a 1-month fill, $200 for 2 months and $300 for 3 months. That applies to both the pen and the pill.
  • Government plans are excluded. That means Medicaid, Medicare, VA, DOD and TRICARE. FEHB, ACA Marketplace and state employee plans don't count as government plans for this offer.
  • Some plan designs block it. Plans with accumulator adjustment, copay maximizer or alternate funding programs aren't eligible.
  • Enroll before you fill. Fills before you enrolled aren't eligible and can't be reimbursed.
  • No stacking. The card can't be combined with self-pay pricing or other coupons. An automatic manufacturer eVoucher at the pharmacy is allowed.

You can text SAVE to 83757 or enroll through NovoCare. For questions, call the program at 1-888-793-1218. Our Wegovy savings card guide covers enrollment step by step.

Whether your plan covers Wegovy, whether the card applies, whether you can pay with HSA or FSA money, and what the IRS allows are four separate questions. A yes to one doesn't answer the others.

Does insurance cover the Wegovy pen, pill and HD the same way?

No. Each form has its own coverage picture:

Does insurance cover the Wegovy pen, pill and HD the same way?
FormInsurance picture (Nov 4, 2026)Self-pay price per month
Wegovy pen, 0.25 mg to 2.4 mgThe usual form plans cover, often with prior authorization; savings card applies if covered$199 for the first 2 months of 0.25 or 0.5 mg (new patients, through Dec 31, 2026), then $349
Wegovy pill, 1.5 mg to 25 mgCard applies if your plan covers it; Ro can't check pill coverage$149 (1.5 mg), $199 (4 mg), $299 (9 mg and 25 mg)
Wegovy HD 7.2 mg penNovoCare: "Currently, no insurance coverage is available for Wegovy® injection 7.2 mg"$399

Ask your plan about each form separately. Our Wegovy pill insurance coverage guide goes deeper.

Do I need prior authorization for Wegovy?

Usually, yes. NovoCare says most plans require prior authorization for Wegovy, that a request can take up to 10 business days, and that an approval can last anywhere from 30 days to over a year. Mark that renewal date. A lapsed approval can stop a refill.

Your prescriber's office files the request. Plans commonly check that your use matches the label: adults with obesity, adults with overweight plus at least one weight-related condition, or adults with heart disease plus obesity or overweight (to reduce heart attack and stroke risk). Some plans also ask you to try other options first. See our step therapy guide.

Many employers also want a lifestyle program. In KFF's 2026 survey, 18% of firms with 200 or more workers covered GLP-1s for weight loss. Of those, 40% required a visit with a dietitian, case manager or therapist, or a lifestyle program.

Carrier rules differ. Check Aetna, Cigna, Blue Cross or UnitedHealthcare for plan-specific steps.

How much is Wegovy with Medicare?

For weight loss, the main route is the Medicare GLP-1 Bridge. CMS runs this temporary program from July 1, 2026 to December 31, 2027, outside the normal Part D benefit.

  • Cost: $50 for a 28- or 30-day supply of any Wegovy pen or pill dose.
  • Who qualifies: You must be 18 or older and have Part D through a drug plan, a Medicare Advantage plan with drug coverage, a Special Needs Plan, an employer group plan or LI NET. You also need one of these: a BMI of 35 or more; a BMI of 30 or more with heart failure with preserved ejection fraction, uncontrolled high blood pressure or chronic kidney disease (stage 3a or higher); or a BMI of 27 or more with prediabetes, a prior heart attack or stroke, or symptomatic peripheral artery disease.
  • Who doesn't qualify: People already getting a GLP-1 through Part D, and people with type 2 diabetes, moderate to severe sleep apnea or fatty liver disease. Medicare says Part D may cover the drug for them instead.
  • The catch: The $50 doesn't count toward your Part D deductible or out-of-pocket limit, and Extra Help can't lower it.
  • Paperwork: Your provider sends the prescription, completes prior authorization if asked, and certifies that you're in a diet and exercise program. Medicare says the approval lasts through December 31, 2027 unless you switch GLP-1s.

If Wegovy is prescribed to reduce heart attack and stroke risk, NovoCare says Part D may cover it the usual way. Medicare.gov says that in 2026 no Part D deductible can exceed $615; after it, you pay 25% coinsurance until your out-of-pocket spending on covered drugs reaches $2,100 for the year. Your plan's drug list shows your actual tier price. The savings card can't be used with Medicare. Our Medicare and Wegovy guide covers plan details.

What if insurance denies Wegovy?

First, find out which kind of no you got. A prior authorization denial can often be fixed. A plan exclusion means the plan doesn't cover weight-loss drugs at all.

NovoCare lists these options:

  1. Appeal. Your doctor can file an appeal or ask for a peer-to-peer review with the plan's reviewer.
  2. Ask your employer. If your coverage comes through work, your doctor can write a letter asking HR to add Wegovy coverage.
  3. Self-pay. Use NovoCare's cash prices while you appeal or if the plan won't budge.

A hypothetical example: a 46-year-old on an employer plan is denied because her chart doesn't show a weight-related condition. Her doctor resubmits with her blood pressure history. That's the kind of denial an appeal can fix; a plan exclusion usually isn't.

Our guides on why insurance denies Wegovy and how to appeal a Wegovy denial walk through the letters and timelines.

If you'd rather not do the phone calls yourself, Ro says its insurance team verifies benefits and handles the paperwork, including prior authorization, which typically takes 1 to 3 weeks. If you're denied, Ro says the team can file an appeal for you, or your provider may try another GLP-1 with a new prior authorization.

Ro's membership is separate from the medication and isn't covered by insurance. Ro lists $39 for the first month (refunded if you're not eligible), then $74 to $149 a month depending on the plan length.

Let Ro's team check your coverage and handle the paperwork (sponsored affiliate link, opens in a new tab)

What does Wegovy cost if you pay cash instead?

If insurance says no, or your insured price is higher than cash, these are Novo Nordisk's self-pay prices as of November 4, 2026:

What does Wegovy cost if you pay cash instead?
Wegovy self-pay (NovoCare)Price per month
Pen 0.25 mg or 0.5 mg, first 2 months for new patients (offer ends Dec 31, 2026)$199
Pen 0.25 mg to 2.4 mg after that$349
Wegovy HD 7.2 mg pen$399
Pill 1.5 mg$149
Pill 4 mg$199
Pill 9 mg or 25 mg$299

Two cautions. A self-pay fill is processed outside your insurance, so it doesn't count toward your deductible or out-of-pocket limit. And the self-pay offer can't be combined with the savings card. People with Medicare or other government coverage can self-pay, but they can't seek reimbursement or count it toward their deductible.

Ro says its Wegovy cash prices match NovoCare's ($199, $349 and $399 for the pen; $149, $199 and $299 for the pill). Ro's membership is extra. For more ways to lower cash costs, see the cheapest ways to get Wegovy without insurance and our Ro Wegovy review.

What changes for Wegovy prices in 2027?

  • Lower list price. Novo Nordisk says it will cut Wegovy's list price to $675 a month on January 1, 2027. That could lower what you pay if your coinsurance or deductible is based on list price. Novo says the cut won't change self-pay prices.
  • Intro offer ends. The $199 pen offer for new patients runs through December 31, 2026.
  • Bridge continues. The Medicare GLP-1 Bridge runs through December 31, 2027.
  • Medicare limits rise. The Part D deductible goes up to as much as $700 and the out-of-pocket cap to $2,400.

Comparing drugs? See our guides to Zepbound cost with insurance, Mounjaro cost with insurance and Ozempic cost with insurance. Ozempic is not FDA-approved for weight loss, so plans that cover it for diabetes may not cover it for weight.

Safety basics before you start

Do not use if you or a family member has had medullary thyroid cancer (MTC), or if you have MEN 2. Wegovy carries a boxed warning about thyroid C-cell tumors. Stop Wegovy at least 2 months before a planned pregnancy, because it may harm an unborn baby. Stop using it and call your healthcare provider right away if you have severe stomach pain that won't go away, which can be a sign of pancreatitis.

What we actually verified

As of November 4, 2026, we opened and checked:

  • Novo Nordisk's Wegovy price pages, savings card terms and coverage page (the $25 card and $100 cap, excluded plans, self-pay prices, prior authorization timing, and the Wegovy HD coverage line).
  • Novo Nordisk's list price page ($1,349.02) and its February 24, 2026 announcement of the $675 list price for 2027.
  • The CMS and Medicare.gov pages on the GLP-1 Bridge and Part D costs.
  • KFF's 2026 Employer Health Benefits Survey.
  • The Wegovy prescribing information (revised 06/2026).

What we didn't verify: your own plan's price. Copays, formularies and accumulator rules vary by plan, so ask your plan or pharmacy.

FAQ

How much is Wegovy with insurance per month?

As little as $25 with a covered commercial plan and the savings card, or $50 through Medicare's GLP-1 Bridge if you qualify. Before your deductible is met, you may owe your plan's full price for the fill.

Why is my Wegovy copay more than $25?

Most likely your plan charges more than $125 a month, so the $100 card cap leaves you with the rest. You may also be in your deductible, or your plan may use an accumulator or copay maximizer, which makes the card ineligible.

Can I use the Wegovy savings card with Medicare?

No. Medicare, Medicaid, VA, DOD and TRICARE members are excluded. People with Medicare who qualify can use the $50 GLP-1 Bridge instead.

Does insurance cover Wegovy HD 7.2 mg?

Not right now, according to NovoCare. Its coverage page says no insurance coverage is currently available for Wegovy injection 7.2 mg. The self-pay price is $399 a month.

How long does Wegovy prior authorization take?

NovoCare says up to 10 business days, and an approval can last from 30 days to over a year.

Is it cheaper to pay cash than use insurance for Wegovy?

Sometimes. If your insured price is above $349 a month for the pen, the self-pay price is lower that month. But cash payments don't count toward your deductible or out-of-pocket limit.

Will Wegovy cost less with insurance in 2027?

Possibly, if your share is based on list price. Novo Nordisk is cutting the list price to $675 on January 1, 2027. Flat copays and self-pay prices aren't tied to that change.
