Cigna Foundayo prior authorization applies when your plan covers the drug under one of Cigna’s public policies. Under the standard rule, adults need 3 months of diet and behavior changes plus a baseline BMI of 30, or 27 with one listed condition. First approval lasts 8 months; renewal needs 5% weight loss. Some plans use stricter rules or exclude weight-loss drugs.
Here’s the part most people miss. Cigna doesn’t use one public BMI rule for Foundayo. It uses three. Your plan picks which one, and nobody hands you a note saying which. We’ll help you find yours in about two minutes. Then we’ll show you exactly what your doctor needs to send so the request doesn’t bounce.
What can change your answer:
- Your plan skips weight-loss drugs. Then a prior authorization can’t fix it. Jump to what to do.

Does Cigna cover Foundayo?
Quick answer: Sometimes. We checked 8 public Cigna drug lists published or active by September 2026. All 8 mark Foundayo for prior authorization and a quantity limit. Six also mark it “OC,” or optional coverage. Seven of the 8 are 2027 lists, so these examples show how Cigna is listing the drug—not what your own 2026 or 2027 plan bought. Your plan documents decide.
First, three quick definitions:
- Prior authorization (PA): Your plan must approve the request before it pays when the PA rule applies.
- Drug list (formulary): Cigna’s list of drugs and coverage notes. On these lists, Tier 2 is the preferred-brand tier.
- OC (optional coverage): Cigna’s legend says most plans do not cover the medicine, but some do. Your employer or plan has to choose the benefit.

What this tells you: Foundayo has PA and quantity-limit controls on every public list we checked. Six of the eight lists make coverage optional. It does not tell you that every Cigna plan covers Foundayo or that every member is under the same rule.
Cigna says the same thing in its policy documents: the member’s benefit-plan document wins if it conflicts with a general coverage policy. That is the damaging admission on this page because it changes everything: perfect medical paperwork cannot create a weight-loss-drug benefit your plan did not buy.
One useful timeline detail: the FDA approved Foundayo on April 1, 2026. Cigna’s IP0621 and IP0739 policies took effect April 30, 2026—29 days later. That is much faster than the “up to six months” review window described in some Cigna drug-list guides. Fast policy creation still does not equal coverage for every plan.

- Log in to the myCigna app or myCigna.com.
- Open Price a Medication.
- Type Foundayo. If nothing comes up, try orforglipron, the active ingredient name.
- Screenshot what you see: covered or not, tier, PA, quantity limit, estimated price, and any program or policy name.
Your pharmacy card may say Express Scripts. That’s normal. Express Scripts, a Cigna company, may manage your prescription benefit. Use the number printed on your own card rather than a general customer-service number.

What are Cigna’s Foundayo prior authorization requirements?
Quick answer: Under Cigna’s standard Foundayo policy, an adult must have tried diet and behavior changes for at least 3 months and have had a baseline BMI of 30 or higher, or 27 or higher with at least one listed weight-related condition. The person must keep up a reduced-calorie diet and behavior changes while taking Foundayo and should not use another GLP-1 at the same time.
The standard checklist
Cigna’s standard public policy allows the first 8 months if your plan includes the benefit and all of these are true:
- ☐ You’re 18 or older.
- ☐ You’ve tried behavior changes and dietary restriction for at least 3 months.
- ☐ Your baseline BMI was 30 or higher, or it was 27 or higher and you have at least one condition from the list below.
- ☐ You’ll keep up the reduced-calorie diet and behavior changes while on Foundayo.
- ☐ You won’t take another GLP-1 receptor agonist at the same time.
BMI (body mass index) is a number from your height and weight. Cigna calls your starting BMI your baseline. For these policies, baseline means before any GLP-1 or GLP-1/GIP medicine. We’ll turn it into pounds in a minute.
The 11 conditions Cigna counts
Cigna’s policy says a listed condition can be present at baseline or currently.

The stricter rules: BMI 32 and BMI 35
This is where people get tripped up. Some Cigna plans use tougher benefit-exclusion override rules. Cigna publishes Foundayo-specific EncircleRx policies at BMI 32 and BMI 35, plus broader drug policies with Foundayo sections at the same thresholds.

The policy number matters. Do not assume every BMI-32 member has the four-weigh-in and four-app-check-in rule. Ask whether Cigna is using IP0621 or the Foundayo-specific CNF1013 policy.
Here’s something Cigna admits in both Foundayo-specific EncircleRx policies: the higher adult BMI lines are “not based on clinical data.” Cigna says those thresholds are part of the benefit offering so a subset of patients can obtain the medication. So if you’re denied at a BMI of 31 under a BMI-32 plan, that is your plan’s benefit rule talking. It is not a new medical finding about you.
Does Cigna make you try other drugs first?
No step therapy appears in the public Foundayo policies we reviewed. Those documents also do not list a specialist requirement or a required lab test for Foundayo. Your exact plan can apply other policies or delegated rules, so confirm the member-specific answer in myCigna or with the number on your card.
Which Cigna rule am I under—and what is my BMI line in pounds?
Quick answer: Your plan picks the rule, not you. Cigna’s standard Foundayo policy uses a BMI line of 30. Stricter BMI 32 and BMI 35 versions exist for some plans, including EncircleRx benefit-exclusion overrides. The chart below turns each BMI line into pounds for common heights.

Our math uses the standard BMI formula: weight in pounds × 703 ÷ height in inches². We rounded up to the next whole pound so the chart does not show a weight that falls just below the line. Your clinician’s dated chart is what Cigna will use.
How to read it: Say you’re 5'6". If you started at 186 pounds or more, you clear the standard line by weight alone. At 168 to 185 pounds, you need at least one listed condition. On a BMI-32 plan, you would need 199 pounds—or at least 168 pounds plus two listed conditions.
Three ways to find out which rule is yours:
- Ask Cigna directly. Use the call script above. Ask for the exact policy ID, not only “standard, 32, or 35.”
- Read your denial or approval letter, if you have one. Look for a policy number or the BMI line used.
- Ask your HR or benefits team whether your plan uses EncircleRx for weight-loss drugs.

Which BMI does Cigna use if I have already lost weight?
Quick answer: Cigna uses your baseline BMI—your BMI before you started any GLP-1 or GLP-1/GIP medicine, such as Wegovy, Zepbound, or Saxenda. If one of those medicines already lowered your weight, the older number is the policy baseline. Your doctor needs a dated record of it.
This one rule saves a lot of people. Here’s an example:
- Before starting Zepbound, you were 5'6" and 220 pounds. That’s a BMI of about 35.5.
- Today you weigh 190 pounds. That’s a BMI of about 30.7.
- For Cigna’s public Foundayo rules, 35.5 is the baseline number. It clears even the BMI-35 line.
Where to find your old weight
If your doctor doesn’t have it, look here:
- Your patient portal’s visit history
- Notes from your primary doctor or specialist
- Records from any telehealth company that prescribed your GLP-1
- A past prior authorization for Wegovy, Zepbound, or Saxenda
- Prescription-claim records
- Lab or visit summaries that list your weight
Ask each office for a copy that shows the date next to the weight. A number without a date is much weaker proof.
The 21-day retail-pharmacy rule for switchers
Cigna’s CNF894 policy includes Foundayo in a retail-pharmacy claim limit. For retail fills of 28 days or more, it allows only one paid claim across the covered weight-loss GLP-1 and GLP-1/GIP medicines every 21 days. The check looks at both retail and home-delivery claim history, and Cigna lists no recommended override.
What that means for you: Say you picked up a 28-day or longer supply of Zepbound at a retail pharmacy on October 1. A retail Foundayo claim may reject before the 21-day window passes. Plan the switch with your doctor and call the number on your card before you go to the pharmacy.
A home-delivery claim is not itself the retail claim that CNF894 limits, but home-delivery history can still count when Cigna checks a later retail claim. Home delivery is not a guaranteed workaround. Ask Cigna how the rule will apply to your exact switch.
Two more switcher facts:
- Do not assume your old approval carries over. Foundayo has its own PA policies. Ask whether a new authorization is required before the first fill.
- Starting your first GLP-1 on cash while you wait? Get your weight recorded before the first dose. Cigna defines baseline as before any GLP-1 or GLP-1/GIP medicine.
What should my doctor send Cigna for Foundayo?
Quick answer: Your doctor’s office usually sends the request. The strongest packet gives Cigna a dated record for every rule: pre-GLP-1 height, weight, and BMI; listed conditions; at least 3 months of diet and behavior notes; and a plan to keep those changes going. Cigna warns that missing details can lead to a denial.
One Foundayo patient summed up the worry on Reddit: “Hoping they fill it out right.” This checklist is how you stop hoping.

How the request gets to Cigna
- Your doctor usually starts it. Cigna says an in-network provider starts the prior authorization. If your provider is out of network, you may need to take a larger role.
- Electronic submission is the normal path. Cigna lists electronic pharmacy-PA options that include Express Scripts tools, Surescripts, and CoverMyMeds.
- Use the right contact. The prescriber should use the submission path tied to your prescription benefit. You should use the prescription-benefit number printed on your own ID card.

How long does Cigna prior authorization take for Foundayo?
Quick answer: Cigna publishes two general estimates. Its 2026 utilization-management flyer says most decisions take 1 to 5 business days. Its general prior-authorization page says 5 to 10 business days after Cigna receives the request. Neither is a Foundayo guarantee. Missing records or a request for more information can add time.
You can track status in the myCigna app or through your doctor’s office. Cigna describes four possible responses:
- Approve the request.
- Deny it.
- Ask for more information.
- Suggest a lower-cost option first.
A common myth: You may read that a federal rule forces a 7-day answer, or 72 hours for an urgent request. CMS-0057-F sets timeframes for certain medical items and services. CMS says those requirements do not apply to prior authorization for drugs. Don’t count on that rule for Foundayo.
Can’t wait? You can ask about paying cash while Cigna decides. A cash purchase may not count toward your plan deductible or out-of-pocket maximum. Ask Cigna before assuming it will. And if this would be your first GLP-1, record your baseline weight first.
How much does Foundayo cost with Cigna?
Quick answer: If your Cigna plan covers Foundayo and you meet Lilly’s savings-card rules, an eligible 1-, 2-, or 3-month fill can cost as little as $25. Without coverage, Lilly’s current self-pay prices run $149 to $349 a month by dose. The savings card and the temporary 2.5 mg and 5.5 mg prices end December 31, 2026 unless Lilly changes or extends them; other self-pay terms can change at any time.

How the $25 savings card really works
Lilly’s commercial savings card requires eligible commercial drug insurance that covers Foundayo. Under terms posted on September 24, 2026:
- An eligible 1-month fill can receive up to $100 in savings.
- An eligible 2-month fill can receive up to $200 in savings.
- An eligible 3-month fill can receive up to $300 in savings.
- The annual maximum is $1,000.
- The limit is 10 fills per calendar year.
- The card expires December 31, 2026, unless Lilly changes or extends it.
For a 1-month fill, the math looks like this:

Our math uses Lilly’s published limits. The card does not guarantee a $25 price. After the card reaches its annual limit, you pay the amount left under your plan.
Money tip: An eligible 60- or 90-day fill can cost as little as $25 for the whole fill, not $25 for each month, as long as the cost and Lilly’s $200 or $300 savings cap allow it. Ask whether your plan permits the longer fill.

What the first 3 months can cost on cash: If your clinician uses the labeled monthly step-up from 0.8 mg to 2.5 mg to 5.5 mg, and you fill all three before December 31, 2026 under the current terms, the medicine total is $149 + $149 + $199 = $497. That is a price example, not dosing advice. After December 31, the current lower 2.5 mg and 5.5 mg prices end unless Lilly extends them.

Can Ro or Sesame help me get Foundayo with Cigna?
Quick answer: Yes, in different ways. Ro says its insurance concierge checks benefits and submits PA paperwork after a clinical visit; membership is $39 for the first month, then $74 to $149 a month depending on the plan. Sesame lets you choose a provider, starts at $59 a month with an annual commitment, and says the provider works with insurance on PA paperwork. Medication costs are separate.
First, the honest part. The best first stop is often your own doctor. They already have your baseline weight and history, which is what Cigna wants to see. A telehealth program makes the most sense if:
- You don’t have a doctor who will prescribe Foundayo or do the paperwork.

Ro’s public checker says it can check Ozempic pens, Wegovy pens, and Zepbound pens—not Foundayo. If a free coverage check before you join anything is your priority, myCigna is better, and it’s free.
Ro’s separate Foundayo page says its insurance concierge checks coverage and submits required paperwork after onboarding when treatment is prescribed. Ro also offers a cash-pay Foundayo path at the manufacturer’s direct price while the insurance process is handled.
Here’s how Ro describes its Foundayo process:
- Eligibility answer: Ro says you can learn whether you are eligible within 2 days after the online visit.
- Paying cash: Ro says the first dose can arrive in less than a week.
- Using insurance: Ro says the insurance process usually takes about 2–3 weeks and its concierge submits PA paperwork when required.
- If insurance says no: Ro says the provider can discuss FDA-approved cash-pay options.
Those are Ro’s published estimates, not guaranteed dates or approval promises.
Ro’s price, plainly: Get started for $39, then as low as $74/month with an annual plan paid upfront. Ro also lists $89/month with a 6-month plan, $99/month with a 3-month plan, and $149 month to month. Medication is a separate charge. Ro says it refunds the first $39 if its clinician finds you are not eligible for a GLP-1 through the program.

Sesame: our pick if choosing your provider matters most
Sesame is a marketplace. You choose the provider you want to see.
- Program price: Success by Sesame starts at $59 a month with a 12-month commitment or $99 month to month. Medication is separate. Sesame says providers set their own prices, so some listings may cost more.
- Insurance help: Sesame says your provider will work directly with insurance on prior-authorization paperwork.
- Included care: Sesame lists video visits, messaging, ongoing care, and labs. Its page says Quest lab costs are not included in Arizona, Hawaii, North Dakota, New Jersey, New York, Oklahoma, Rhode Island, South Dakota, or Wyoming.
- Foundayo pricing: Sesame’s main Foundayo table shows regular dose prices. Confirm the current manufacturer offer and final checkout price before paying.

Why did Cigna deny Foundayo—and what do I do now?
Quick answer: Most problems fall into three groups: the clinical rule was not met or not proven, the pharmacy claim hit an administrative rule, or the plan excludes the benefit. The right response depends on which one happened. Better BMI records can fix a baseline mistake. They usually cannot turn a true plan exclusion into coverage.

How to respond to a denial
- Get the full written denial. It should list the reason, appeal rights, and deadline.
- Sort it: clinical rule, missing paperwork, pharmacy rule, or benefit exclusion?
- Ask for the exact policy and criterion Cigna used.
- Compare it with your records. Find the gap.
- Correct and resubmit, or appeal using the method in the letter.
- Meet the deadline in your letter. Cigna’s public employer-plan page gives 180 calendar days as a general appeal window, but it also says state rules and plan terms can differ. Use the deadline on your own notice.
- Keep copies of everything, with dates and confirmation numbers.
Some plans offer an external review—an independent reviewer outside Cigna—after the internal appeal, especially when the decision involves medical judgment. Your notice and plan documents control whether it is available.

What if my Cigna plan excludes Foundayo?
Quick answer: If your plan excludes weight-loss drugs, Cigna’s medical rules do not create coverage even when you meet them. Ask whether an EncircleRx benefit-exclusion override or exception process applies, check what may change at the next open enrollment, or use a cash-pay path. Lilly’s current Foundayo self-pay price starts at $149 a month.
Ask these three questions first
- Is Foundayo itself non-covered, or are all weight-loss drugs excluded? Those are different problems.
- Does my plan use EncircleRx for weight-loss drugs? If yes, a BMI-32 or BMI-35 override may be your coverage path.
- Does my plan allow an exception request? Your plan documents or the number on your card can tell you.
Your options, in order
- Ask HR or benefits. Employers often choose whether optional weight-loss coverage is included. Ask for your Summary Plan Description and whether the benefit may change at the next open enrollment.
- Use Lilly’s self-pay path. The current starting-dose price is $149. See the price table.
- Use a telehealth program if you also need a prescriber: Ro or Sesame.

What does Cigna need to renew Foundayo?
Quick answer: Cigna’s first Foundayo approval lasts 8 months. To renew for one year, you must have lost at least 5% of baseline weight—measured from before any GLP-1—and still be following a reduced-calorie diet and behavior changes. Someone with a 250-pound baseline needs a current weight of 237.5 pounds or less.
Most people expect a full year. Cigna’s general drug flyer says medicines are typically approved for a year. Foundayo’s first public-policy approval is shorter, so plan ahead.

Mark your calendar the day you’re approved. Cigna defines a policy month as 30 days, so 8 policy months equals 240 days. Your approval letter controls the real start and end dates. Ask when Cigna will accept the renewal, and get an updated weight on file before the authorization ends.
On a BMI-32 or BMI-35 policy? The Foundayo-specific EncircleRx policies expressly require patient-specific proof of baseline BMI, listed conditions, and the 5% renewal loss. IP0621 also has the separate vendor-engagement rule when that exact policy applies.

What changes for Medicare, individual plans, diabetes, and birth control?
Quick answer: The Cigna rules on this page are commercial-plan policies. Medicare has a separate CMS program. Eligible Medicare Part D members can get Foundayo for weight management through the Medicare GLP-1 Bridge for a fixed $50 copay per 28- or 30-day fill through December 31, 2027. Prior authorization and Bridge eligibility still apply.
- Medicare. The Bridge runs outside the normal Part D coverage and payment flow. The $50 does not count toward the Part D deductible or true out-of-pocket spending, and Lilly’s commercial savings card cannot lower it. See Does Medicare cover Foundayo?
- Individual and family plans. Cigna’s policy documents warn that benefit-plan terms may exclude weight-loss medicine. Check myCigna the same way.
- Type 2 diabetes. Foundayo is FDA-approved for weight management, not as a diabetes treatment. The label says concurrent use with another GLP-1 receptor agonist is not recommended. That includes medicines such as Ozempic, Mounjaro, Trulicity, and Rybelsus.
- Birth control pills. The label says Foundayo can affect oral contraceptive absorption. Switch to a non-oral method or add a barrier method for 30 days after starting and 30 days after each dose increase.

Is Foundayo safe to ask for? What does the FDA label say?
Quick answer: Foundayo is an FDA-approved once-daily GLP-1 pill for adults with obesity, or overweight plus a weight-related condition, used with a reduced-calorie diet and increased physical activity. Its label has a boxed warning about thyroid C-cell tumors. A Cigna approval is a coverage decision, not proof that the medicine is right for you.
What to know before you ask your doctor:
- How you take it: One whole tablet a day, with or without food. The labeled starting dose is 0.8 mg, followed after at least 30 days by 2.5 mg, then after at least 30 more days by 5.5 mg. A clinician may increase it to 9, 14.5, or 17.2 mg after at least 30 days at each level based on response and tolerability.
- Concurrent GLP-1 use: The label says taking Foundayo with another GLP-1 receptor agonist is not recommended.
- Boxed warning: thyroid C-cell tumors. Do not take it if you or a family member has had medullary thyroid carcinoma, or if you have Multiple Endocrine Neoplasia syndrome type 2.
- Other label warnings include pancreatitis, severe stomach and gut reactions, kidney injury from dehydration, low blood sugar in some patients, serious allergic reactions, diabetic retinopathy complications, gallbladder disease, and pulmonary aspiration during anesthesia or deep sedation.
- Not recommended for people with severe gastroparesis or severe liver impairment.
- Pregnancy: The label says Foundayo may cause fetal harm and should be stopped when pregnancy is recognized.
- Most common side effects reported in at least 5% of patients include nausea, constipation, diarrhea, vomiting, indigestion, abdominal pain, headache, bloating, fatigue, burping, reflux, gas, and hair loss.

Cigna Foundayo prior authorization FAQ
Does Cigna cover Foundayo?
Sometimes. Cigna publishes Foundayo coverage policies, and all eight public drug-list examples we checked mark the drug for PA and a quantity limit. Six also mark it optional coverage. Your member-specific benefit—not the public list alone—decides whether the plan includes Foundayo.
Does Cigna require prior authorization for Foundayo?
Cigna’s public Foundayo policies use prior authorization for covered requests, and every public drug list we checked marks Foundayo “PA.” The exact member record still controls because Cigna says multiple policies may apply and the plan document wins.
What BMI does Cigna require for Foundayo?
Under the standard rule, baseline BMI must be 30 or more, or 27 or more with one listed condition. Some benefit-exclusion override policies use BMI 32 or 35, or BMI 27 with two listed conditions.
Does Cigna use my current BMI or my baseline BMI?
Cigna uses baseline BMI from before any GLP-1 or GLP-1/GIP medicine. If Wegovy or Zepbound already lowered your weight, the public policies point back to the older, pre-treatment BMI.
Does PCOS count as a condition for Cigna?
Yes. Polycystic ovarian syndrome is on Cigna’s list of 11 weight-related conditions. The standard BMI-27 path needs at least one listed condition. The public BMI-32 and BMI-35 override paths need at least two.
Do I need to join a formal weight-loss program first?
The standard Foundayo policies ask for at least 3 months of behavior changes and dietary restriction, not a named commercial program. IP0621 also lists four weigh-ins and four app engagements each month, but the Foundayo-specific CNF1013 BMI-32 policy does not. Ask which policy ID applies.
Does Cigna make me try other drugs before Foundayo?
The public Foundayo policies we reviewed do not list step therapy. They also do not list a specialist requirement or required lab test. Your exact plan may apply other policies, so confirm the member-specific answer.
How long does Cigna take to decide?
Cigna publishes two general ranges: 1 to 5 business days in its 2026 utilization-management flyer and 5 to 10 business days on its general prior-authorization page. Those are estimates, not a Foundayo guarantee. Missing information can add time.
How long does a Cigna Foundayo approval last?
The public Foundayo policies give 8 months for initial therapy. Cigna defines a policy month as 30 days. Continuation approval is 1 year when the renewal criteria are met. Your approval letter controls the real dates.
What do I need to renew Foundayo with Cigna?
You need at least 5% weight loss from the pre-GLP-1 baseline, plus continued behavior changes and a reduced-calorie diet. If the baseline was 200 pounds, a 5% loss means 190 pounds or less.
Can I take Foundayo with Wegovy, Zepbound, Ozempic, or another GLP-1?
The FDA label says concomitant use with another GLP-1 receptor agonist is not recommended. Cigna’s Foundayo policies also list concurrent GLP-1 use as not covered or not medically necessary under those policies.
What is Cigna’s 21-day Foundayo rule?
For qualifying retail fills of 28 days or more, CNF894 allows one paid claim across the listed weight-loss GLP-1 and GLP-1/GIP medicines every 21 days. It checks retail and home-delivery history. Home delivery is not a guaranteed workaround for a later retail rejection.
Can I use the Foundayo savings card with Cigna?
Yes, if you have eligible commercial drug insurance and Cigna covers Foundayo. Under current terms, savings are capped at $100 for a 1-month fill, $200 for a 2-month fill, and $300 for a 3-month fill, with a $1,000 yearly limit and 10-fill limit. The current card expires December 31, 2026.
How much is Foundayo if Cigna will not cover it?
Lilly’s current self-pay prices run from $149 to $349 a month by dose. Through December 31, 2026, the 2.5 mg dose is $149 and the 5.5 mg dose is $199. The 14.5 mg and 17.2 mg doses can be $299 when the 45-day offer terms are met; otherwise they are $349.
Can Ro check my Cigna Foundayo coverage for free?
Not with Ro’s public free checker. Ro says that tool cannot check Foundayo right now. After a person joins and treatment is prescribed, Ro’s Foundayo page says its insurance concierge checks coverage and submits PA paperwork when needed.
What if Cigna says Foundayo is excluded?
Ask whether only Foundayo is non-covered or all weight-loss drugs are excluded, whether an EncircleRx override applies, and whether your plan allows an exception. A normal PA cannot by itself add a benefit your plan excludes. Lilly’s current self-pay price starts at $149 a month.
Does Medicare cover Foundayo?
Eligible Medicare Part D members can access Foundayo for weight management through the Medicare GLP-1 Bridge. The copay is $50 for a 28- or 30-day fill, prior authorization applies, and the program runs through December 31, 2027. Not every Medicare member qualifies.
