Mounjaro Cost at Costco: What You'll Pay in 2026

By The RX Index Editorial Team · Last verified

Mounjaro cost at Costco is $1,103.95 for a box of four pens, the same at every strength from 2.5 mg to 15 mg, on Costco's Member Value Pricing search as of November 5, 2026. That is cash. Lilly's list price is $1,112.16 per fill; LillyDirect self-pay starts at $499 a month — about half of Costco's cash number.

You need a prescription either way. Mounjaro is FDA-approved for type 2 diabetes, not for weight loss. The tables below show Costco's price, what a year costs, how insurance and Lilly's savings card change the bill, and when LillyDirect or a weight-loss tirzepatide path beats Costco. We checked every price on its source page on November 5, 2026.

The RX Index is the independent GLP-1 decision resource that scores telehealth providers and treatment paths on clinical legitimacy, care quality, transparency, access, and cost, so readers can choose the path that fits their situation.

Decision Verdict

Costco is a convenient place to fill brand-name Mounjaro, but it is not the cheap cash path. Its published member cash price sits near Lilly's list price. If you are paying cash for Mounjaro, LillyDirect's $499 self-pay is the lower published number. If your commercial plan covers Mounjaro, the savings card at as little as $25 will usually beat both.

  • Best for: Adults (or eligible teens 10+) with type 2 diabetes who already use Costco Pharmacy, have a Mounjaro prescription, and either have coverage plus the savings card or simply want to confirm Costco's cash quote before transferring a script.
  • Not ideal for: Anyone paying cash who can use LillyDirect's $499 self-pay instead, anyone whose goal is weight loss rather than type 2 diabetes (ask about Zepbound), and anyone expecting Costco's Wegovy/Ozempic Member Value deals to apply to Mounjaro — those featured cards are different drugs.
  • The factor that matters most: Whether the fill is billed to commercial insurance with Lilly's savings card, run as Costco cash, or filled through LillyDirect self-pay. Those are three different prices.

How much does Mounjaro cost at Costco without insurance?

Without insurance, Costco's Member Value Pricing tool lists Mounjaro at $1,103.95 for one box of four pens. That price showed for every strength we checked: 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg and 15 mg. Costco's tool showed the same $1,103.95 for warehouse/network pickup and for Costco Mail Order. One box is one month's supply (Lilly defines a month as 28 days and up to four pens).

Costco's Member Value Pricing landing page features Wegovy and Ozempic deals. It does not put a Mounjaro card on that homepage. The $1,103.95 figure comes from Costco's own drug-price search for Mounjaro, not from those featured GLP-1 cards. Confirm the quote for your warehouse before you pay — pharmacy cash prices can move.

Your situation changes which of these prices you'll actually pay. The right GLP-1 path depends on your state, your insurance and formulary, FDA-approved brand vs other options, shot or pill, and your budget. The RX Index's Find My GLP-1 Path tool sorts that out in about 2 minutes, with no signup.

Is Mounjaro cheaper at Costco than LillyDirect?

No — not on the published cash numbers. Costco's Member Value cash price is $1,103.95. LillyDirect lists Mounjaro self-pay starting at $499 a month at every pen strength, with a prescription. TrumpRx shows the same math against Lilly's $1,112.16 list price (about 55% off, eligibility required) and sends you through LillyDirect for savings and delivery.

So the reason to choose Costco is convenience or insurance workflow, not a lower cash sticker. If you already fill at Costco and your plan or savings card works there, stay. If you are paying cash for Mounjaro, LillyDirect's $499 is the lower published self-pay price. See also our broader Mounjaro cost without insurance guide and NovoCare vs LillyDirect.

LillyDirect lists Amazon Pharmacy, Fuze Health, Gifthealth and Walmart Pharmacy for fulfillment — not Costco. It also says it can support retail pickup near you; ask LillyDirect before assuming your Costco qualifies.

Do you need a Costco membership to get Mounjaro at Costco?

No membership is required to use Costco Pharmacy online or at warehouses, per Costco's FAQ. Costco still brands this as Member Value Pricing, a member savings program, not insurance. A $65 basic membership bought only for cash-pay Mounjaro does not bring the drug down to LillyDirect's $499.

Does the Costco Executive membership pay for itself on Mounjaro?

Only a little, and only if you are already paying Costco cash. Executive members pay an extra $65 a year for a 2% reward. Costco says pharmacy purchases count "in most cases," using only your out-of-pocket share, and not in some states (including Arkansas, New Jersey and New York). On $14,351.35 of Costco cash Mounjaro, 2% is about $287 — roughly $222 after the upgrade fee (Costco Executive terms, November 5, 2026). That still leaves you far above LillyDirect's $6,487 self-pay year.

How much is Mounjaro at Costco with insurance?

Costco's $1,103.95 Member Value price is cash only. Costco says claims sent to private insurance, Medicare, Medicaid or other public programs aren't eligible for Member Value Pricing. If you have coverage, ask Costco to run the fill through your plan instead of Member Value Pricing.

If you have commercial insurance that covers Mounjaro: Use your coverage and Lilly's Mounjaro Savings Card. Lilly says eligible commercially insured patients may pay as little as $25 for up to a 3-month prescription, with savings capped at $150 per 1-month fill, $300 per 2-month fill or $450 per 3-month fill, and up to $1,950 per calendar year (up to 13 fills). Government beneficiaries are excluded. That path usually beats $1,103.95. See Mounjaro cost with insurance.

If you have commercial insurance that does not cover Mounjaro: Lilly's savings card has a separate tier: as low as $499 for a 1-month fill (28 days, up to four pens), with maximum monthly savings of $647 and maximum annual savings of $8,411, through December 31, 2026. That $499 card price matches LillyDirect's published self-pay figure, but the card still requires commercial insurance that excludes coverage — it is not for people with no insurance.

If your plan covers Mounjaro but your share is more than $1,103.95: Paying Costco cash that month could cost less on the receipt. The catch is that cash Member Value payments don't count toward your deductible or out-of-pocket limit.

If you have Medicare: Lilly's savings card excludes government beneficiaries. Lilly says Mounjaro is covered on many Medicare Part D plans for type 2 diabetes — check your plan's copay first. Medicare's GLP-1 Bridge ($50 a month starting July 1, 2026) covers Foundayo, Wegovy and the Zepbound KwikPen, not Mounjaro. See Medicare GLP-1 Bridge at Costco.

If you have Medicaid, VA, TRICARE or another government plan: The savings card does not apply. Check that plan's rules before you pay Costco cash.

HSA or FSA: Costco says it accepts HSA and FSA cards at its registers. Your plan decides what's eligible; see using an HSA or FSA for Ozempic for the same HSA/FSA questions that apply to diabetes GLP-1 fills.

Not sure what your plan covers? Ro's insurance team will check coverage for Ozempic, Wegovy and Zepbound pens and handle the paperwork, and Ro says the check is free. Ro does not currently offer Mounjaro itself — its Mounjaro page says so — but the coverage check is still useful when you are deciding whether to fight for tirzepatide coverage or pay cash.

Want Mounjaro for weight loss? Check Zepbound's cash price first

Mounjaro is FDA-approved to improve blood sugar in adults and in pediatric patients 10 and older with type 2 diabetes, and to lower the risk of major cardiovascular events in high-risk adults with type 2 diabetes. It is not FDA-approved for weight management. Zepbound is Lilly's tirzepatide brand approved for weight management.

If weight loss is your goal, a Costco cash fill of Mounjaro at $1,103.95 is usually the wrong comparison. Ask a prescriber about Zepbound. LillyDirect and Ro both publish Zepbound cash prices far below Costco's Mounjaro cash quote. On Ro's pricing page on November 5, 2026, Zepbound KwikPen cash prices start at $299 for 2.5 mg, $399 for 5 mg, and $449 for 7.5 mg through 15 mg with the manufacturer offer (Ro also shows a higher starting list figure). Ro membership is $39 the first month, then $74 to $149 a month depending on the plan — medication is separate. Details: Zepbound vs Mounjaro, LillyDirect Zepbound vial vs insurance, does Ro have tirzepatide and Ro GLP-1 cost.

For Costco shoppers comparing other GLP-1 cash deals Costco does feature, see Ozempic cost at Costco and how much Wegovy costs at Costco. Those Member Value cards are not Mounjaro prices.

How do you get a Mounjaro prescription filled at Costco?

Costco needs a valid prescription. Ask your doctor to send it to your Costco pharmacy, or ask Costco to transfer it. Some telehealth services can prescribe Mounjaro for type 2 diabetes when appropriate — confirm they will send the script to Costco before you pay a visit fee (how to get Mounjaro online). Costco dispenses the same FDA-approved Lilly product as any licensed pharmacy, not compounded tirzepatide.

What to say at the Costco pharmacy counter

  • "Please quote Member Value cash and, separately, my insurance with the Lilly savings card if I'm eligible."
  • "Is $1,103.95 still the cash price for my strength today, for a box of four pens?"
  • "Please do not run Member Value Pricing if we are billing my commercial plan."
  • "Is it in stock, and when can I pick it up?"

Safety before you fill

Do not use Mounjaro if you or a family member has had medullary thyroid cancer (MTC), or if you have multiple endocrine neoplasia syndrome type 2 (MEN 2). Stop and call your doctor if you get severe stomach pain that will not go away, which can be a sign of pancreatitis. If you are pregnant or planning a pregnancy, talk with your prescriber first; the label says Mounjaro may cause fetal harm based on animal studies and should be used in pregnancy only if the potential benefit justifies the potential risk.

Frequently asked questions

How much is Mounjaro at Costco without insurance?

Costco's Member Value Pricing search listed $1,103.95 for a box of four pens on November 5, 2026, at every strength from 2.5 mg to 15 mg. Lilly's list price is $1,112.16 per fill. LillyDirect's self-pay price starts at $499 a month.

Is Mounjaro cheaper at Costco than other pharmacies?

Costco's published cash price is near list. It is not lower than LillyDirect's $499 self-pay. Other retail cash quotes vary by ZIP and discount card; we did not verify GoodRx or similar Costco coupons today. Compare any quote with $1,103.95 and with $499.

Does Costco take the Mounjaro savings card?

Lilly's savings card is for eligible commercially insured patients and is meant to be used at the pharmacy with your claim. Costco's Member Value cash price cannot be combined with insurance claims. Ask your Costco pharmacist to run the card with your commercial plan when you are eligible. People on Medicare, Medicaid and other government plans cannot use the card.

Do I need a Costco membership for Mounjaro?

Costco says you do not need a membership to buy pharmacy prescriptions online or at its warehouses. Member Value Pricing is still described as a member savings program, so ask your warehouse how non-member cash fills are priced if you are not a member.

Can I get Mounjaro at Costco for weight loss?

Mounjaro is FDA-approved for type 2 diabetes, not weight loss. If weight loss is your goal, ask a prescriber about Zepbound. Costco's featured Member Value deals for Wegovy and Ozempic are separate products with their own prices.

Is Costco's Mounjaro price the same as LillyDirect?

No. Costco cash was $1,103.95; LillyDirect self-pay starts at $499 a month. LillyDirect's listed dispensing pharmacies do not include Costco.

Does the Costco Executive 2% reward help on Mounjaro?

It can shave about $222 a year off a full year of Costco cash fills after the $65 upgrade, if pharmacy purchases count in your state. It does not bring Costco cash down to LillyDirect's $499.
