Zepbound cost at Costco depends on the device. On Costco's Member Value Pricing search as of November 6, 2026, single-dose pens are $539.30 for a box of four at every strength from 2.5 mg to 15 mg. Single-dose vial 4-packs start at $149.73. KwikPens start at $495.45. Those are cash prices. TrumpRx lists the LillyDirect vial self-pay path at $299 to $449 off a $1,087 original price.
You need a prescription either way. Zepbound is FDA-approved for weight management in adults, and for moderate to severe obstructive sleep apnea in adults with obesity — not for type 2 diabetes under this brand name (that tirzepatide brand is Mounjaro). The tables below show Costco's prices by device, what a year costs, how insurance changes the bill, and when LillyDirect or Ro beats Costco cash. We checked every price on its source page on November 6, 2026.

Decision Verdict
Costco is a convenient place to fill brand-name Zepbound, but the cheapest cash path depends on the device. Costco's published vial quotes for some starter strengths sit below the TrumpRx/LillyDirect vial ladder. Costco's KwikPen cash quotes sit above Ro's and LillyDirect's published KwikPen offer prices. If your commercial plan covers the single-dose pen, Lilly's savings card at as little as $25 usually beats both.
- Best for: Adults with a Zepbound prescription who already use Costco Pharmacy and want a dated cash quote by device before transferring a script, or who have coverage plus the savings card and want Costco to bill the plan (not Member Value cash).
- Not ideal for: Anyone assuming Costco's featured Wegovy/Ozempic Member Value cards apply to Zepbound — those homepage cards are different drugs. Also a poor fit if you want the lowest published KwikPen cash price (Ro and LillyDirect's offer ladder beat Costco's KwikPen quotes today).
- The factor that matters most: Whether the fill is a covered commercial pen with Lilly's savings card, Costco Member Value cash for a specific device, or LillyDirect/Ro self-pay. Those are different prices.
How much does Zepbound cost at Costco without insurance?
Without insurance, Costco's Member Value Pricing tool returns different cash prices by device. One month is generally four weekly doses (a 4-pen carton, a 4-vial carton, or one KwikPen with four doses). Costco's Member Value Pricing landing page features Wegovy and Ozempic deals. It does not put a Zepbound card on that homepage. The figures below come from Costco's own price API for each Zepbound NDC on November 6, 2026 — confirm the quote for your warehouse before you pay.

Your situation changes which of these prices you'll actually pay. The right GLP-1 path depends on your state, your insurance and formulary, FDA-approved brand vs other options, shot device, and your budget. The RX Index's Find My GLP-1 Path tool sorts that out in about 2 minutes, with no signup.

Is Zepbound cheaper at Costco than LillyDirect?
It depends on the device — and you should compare apples to apples.
TrumpRx lists Zepbound vials with an original price of $1,087.00 and cash prices of $299 (2.5 mg), $399 (5 mg), and $449 (7.5 mg through 15 mg), eligibility required. TrumpRx says Eli Lilly manages those vial orders through LillyDirect. Ro's pricing page on November 6, 2026, lists Zepbound KwikPen cash at $299 / $399 / $449 with the manufacturer offer and says its cash prices match LillyDirect, NovoCare and TrumpRx.
| Where you fill | Published cash price (28-day supply) | Notes |
|---|---|---|
| Costco — single-dose pens (4) | $539.30 any strength | Member Value search; not on MVP homepage cards |
| Costco — vials (4) | $149.73 to $349.73 by strength | Below TrumpRx/LillyDirect for some starter strengths — confirm at the counter |
| Costco — KwikPen | $495.45 (2.5–7.5 mg) or $693.93 (10–15 mg) | Higher than Ro/LillyDirect offer ladder today |
| TrumpRx / LillyDirect vials | $299 / $399 / $449 | Original $1,087; routes through LillyDirect |
| Ro — Zepbound KwikPen | $299 / $399 / $449 + Ro membership | Medication separate from $39 first-month membership |

So the reason to choose Costco is convenience, a specific device quote, or insurance workflow — not a single "Costco is always cheapest" claim. Do you need a Costco membership to get Zepbound at Costco?
No membership is required to use Costco Pharmacy online or at warehouses, per Costco's FAQ. Costco still brands this as Member Value Pricing, a member savings program, not insurance. A $65 basic membership bought only for cash-pay Zepbound does not automatically match LillyDirect's vial ladder — compare the device quote you need.
Does the Costco Executive membership pay for itself on Zepbound?
Only a little, and only if you are already paying Costco cash. Executive members pay an extra $65 a year for a 2% reward. Costco says pharmacy purchases count "in most cases," using only your out-of-pocket share, and not in some states (including Arkansas, New Jersey and New York). On $7,010.90 of Costco cash pens, 2% is about $140 — roughly $75 after the upgrade fee (Costco Executive terms, November 6, 2026). That still may not beat a lower LillyDirect or Ro cash path for your device.
How much is Zepbound at Costco with insurance?
Costco's Member Value prices above are cash only. Costco says claims sent to private insurance, Medicare, Medicaid or other public programs aren't eligible for Member Value Pricing. If you have coverage, ask Costco to run the fill through your plan instead of Member Value Pricing.
If you have commercial insurance that covers Zepbound single-dose pens: Lilly's pricing page says eligible commercially insured patients may pay as little as $25 for a one- or three-month supply of single-dose pens (month = 28 days, up to four pens). Governmental beneficiaries are excluded. Terms apply and can change. That path usually beats $539.30 cash. See Zepbound cost with insurance and Zepbound savings card.
If your plan covers Zepbound but your share is more than Costco cash for your device: Paying Costco cash that month could cost less on the receipt. The catch is that cash Member Value payments don't count toward your deductible or out-of-pocket limit.
If you have Medicare: Lilly's commercial savings card excludes government beneficiaries. Medicare's GLP-1 Bridge ($50 a month starting July 1, 2026) covers the Zepbound KwikPen only — not single-dose pens or vials. See Medicare GLP-1 Bridge at Costco.
If you have Medicaid, VA, TRICARE or another government plan: Commercial savings-card terms usually exclude you. Check that plan's rules before you pay Costco cash.
HSA or FSA: Costco says it accepts HSA and FSA cards at its registers. Your plan decides what's eligible; see using an HSA or FSA for Ozempic for the same HSA/FSA questions that apply to GLP-1 fills.
Not sure what your plan covers? Ro's insurance team will check coverage for Ozempic, Wegovy and Zepbound autoinjector pens and handle the paperwork, and Ro says the check is free.

How do you get a Zepbound prescription filled at Costco?
Costco needs a valid prescription. Ask your doctor to send it to your Costco pharmacy, or ask Costco to transfer it. Costco dispenses the same FDA-approved Lilly product as any licensed pharmacy, not compounded tirzepatide. Compounded drugs are not FDA-approved.
What to say at the Costco pharmacy counter
- "Please quote Member Value cash for my exact device — pen, vial or KwikPen — and, separately, my insurance with the Lilly savings card if I'm eligible."
- "Is today's cash price still $539.30 for pens / the vial or KwikPen quote for my strength?"
- "Please do not run Member Value Pricing if we are billing my commercial plan."
- "Is it in stock, and when can I pick it up?"

Which path fits you
| Your situation | Likely lowest published path | Next step |

| Commercial plan covers Zepbound pens | Copay + Lilly savings card (as little as $25) | Ask Costco to bill insurance, not Member Value |
| Paying cash, want vials | Compare Costco vial quote with TrumpRx/LillyDirect $299–$449 | Confirm both quotes before you transfer |
| Paying cash, want KwikPen | Ro / LillyDirect offer ladder ($299–$449) often beats Costco's $495–$694 | Check Ro cash prices |
| Not sure what your plan covers | Find out before you pay cash | Ro's free coverage check |
| Medicare Bridge eligible | $50 KwikPen via Bridge | Pens/vials not in Bridge |
| Already love Costco Pharmacy and have coverage | Costco + savings card | Call your warehouse with the counter script |

Safety before you fill
Do not use Zepbound if you or a family member has had medullary thyroid cancer (MTC), or if you have multiple endocrine neoplasia syndrome type 2 (MEN 2). Stop and call your doctor if you get severe stomach pain that will not go away, which can be a sign of pancreatitis. If you are pregnant or planning a pregnancy, talk with your prescriber first; the label says Zepbound may cause fetal harm and to discontinue when pregnancy is recognized.

- Ran Costco's Member Value Pricing price check for Zepbound pen, vial and KwikPen NDCs on November 6, 2026, and recorded every networkPrice above.
- Confirmed Costco's MVP landing page features Wegovy and Ozempic, not Zepbound.
- Read TrumpRx's Zepbound vial page (original $1,087; cash $299 / $399 / $449; LillyDirect fulfillment).
- Read Lilly's Zepbound pricing-info page for the "as low as $25" commercial pen language (list-price numeral was blank in the text fetch).

What we could not confirm: stock at your warehouse, whether your Costco will successfully process Lilly's savings card on your exact claim, any Costco fee beyond the published Member Value price, and LillyDirect's own HTML (Cloudflare-blocked today — cash vial ladder taken from TrumpRx). Confirm the price at the counter before you pay.
Frequently asked questions
How much is Zepbound at Costco without insurance?
On November 6, 2026, Costco's Member Value price check listed single-dose pens at $539.30 for a box of four at every strength, vials from $149.73 to $349.73 for a 4-pack, and KwikPens from $495.45 to $693.93. Confirm your device and warehouse quote before you pay.
Is Zepbound cheaper at Costco than LillyDirect?
For some Costco vial strengths, Costco's published cash quote is lower than TrumpRx/LillyDirect's $299–$449 ladder. For KwikPens, Costco's cash quotes are higher than Ro's and LillyDirect's published offer prices ($299–$449). Compare the exact device.
Does Costco take the Zepbound savings card?
Lilly's commercial savings card is meant to be used at the pharmacy with your insurance claim. Costco's Member Value cash price cannot be combined with insurance claims. Ask your Costco pharmacist to run the card with your commercial plan when you are eligible. People on Medicare, Medicaid and other government plans generally cannot use the commercial card.
Do I need a Costco membership for Zepbound?
Costco says you do not need a membership to buy pharmacy prescriptions online or at its warehouses. Member Value Pricing is still described as a member savings program, so ask your warehouse how non-member cash fills are priced if you are not a member.
Does Costco's homepage advertise a Zepbound deal?
Not on November 6, 2026. The Member Value Pricing landing page featured Wegovy and Ozempic cards. Zepbound prices came from Costco's drug price check, not those featured cards.
Is Costco's Zepbound the same as compounded tirzepatide?
No. Costco Pharmacy dispenses FDA-approved brand Zepbound. Compounded tirzepatide is not FDA-approved.
Does the Costco Executive 2% reward help on Zepbound?
It can shave a small amount off a year of Costco cash fills after the $65 upgrade, if pharmacy purchases count in your state. It does not by itself make Costco beat a lower LillyDirect or Ro cash path for your device.
