Does Cigna Cover Saxenda? Brand, Generic, and Cigna's Rules for 2026–2027

By The RX Index Editorial Team · Last verified

Does Cigna cover Saxenda? Sometimes. Your plan must include weight-loss drugs and approve the product. Under Cigna's standard adult policies, first approval lasts 4 months; renewal requires at least 4% weight loss plus other criteria. The employer-plan rule for starting brand Saxenda also requires trying its generic. Check which policy your plan uses.

And there's a bigger change coming. Novo Nordisk, which makes Saxenda, is discontinuing the brand, with last shipments to wholesalers in late January 2027. Below: what that means for you, the exact rules Cigna uses, and a quick check of where you stand.

RX Index Decision Verdict: Check Saxenda and its weight-loss generic in myCigna before anything else, and start the coverage request through your own prescriber.

  • Best for: Adults with a Cigna employer plan who've been prescribed Saxenda or its generic, or who are already taking it.
  • Not ideal for: Medicare, Medicaid, or anyone under 18. Those rules are different; see the plan-type section.
  • The factor that matters most: Whether your plan includes weight-loss drugs at all. Paperwork can't add a benefit your plan doesn't have.

We can't see your plan or promise your copay. What we can do is show you Cigna's own rules, in plain words, and the exact questions that get you a real answer.

What your Cigna result actually means

A Cigna answer about Saxenda can involve your benefit, the product, approval records, renewal, or a pharmacy-processing issue. Each one needs a different next step. Find the line that matches what you saw or heard.

What your Cigna result actually means

You only checked a public drug list, or you're not sureYour personal coverage is still unknown.Use myCigna or the pharmacy number on your card. Unknown doesn't mean "not covered."

Three terms, defined once:

  • Formulary: your plan's drug list.
  • Prior authorization (PA): a permission slip from Cigna. Your doctor's office sends your records, and Cigna decides before it pays.
  • Benefit exclusion: the plan simply leaves weight-loss drugs out. That's different from a request that's missing paperwork.

For an answer tied to your own plan, use Cigna's member tool. Log in at myCigna, open Price a Medication, and search both "Saxenda" and "liraglutide." Cigna's drug lists point members to that tool for coverage and costs on their own plan (Cigna Standard 3-Tier list).

Saxenda vs. generic liraglutide: which one Cigna wants

Saxenda's FDA-approved generic is the same medicine, liraglutide, made by Teva and approved on August 27, 2025. For initial treatment with brand Saxenda, Cigna's employer-plan rule requires a generic trial and a documented inactive-ingredient difference that would cause a significant allergy or serious reaction. There's also a different liraglutide generic, the one for Victoza, which is a type 2 diabetes product.

Liraglutide, generic of VictozaA diabetes product (FDA)Not the weight-loss product. A "liraglutide covered" result could mean this one.

Here's the catch with brand Saxenda. Under Cigna's employer-plan policy IP0813 (effective September 1, 2026), initial brand approval requires that you tried the generic and your prescriber documents that an inactive-ingredient difference would cause a significant allergy or serious reaction. "The brand works better for me" isn't on that list. This is the initial employer-plan rule; renewal has a separate set of requirements.

Cigna's change notice shows this shift started earlier. On December 15, 2025, Cigna moved Saxenda to a higher tier and added prior authorization on its Standard and Value drug lists, naming "liraglutide (SAXENDA generic)" as the lower-cost choice (Cigna 2025 coverage changes).

Say your myCigna search shows Saxenda as "not covered" but liraglutide as "PA required." That's worth checking before treating it as a "no." Ask the pharmacist: "Is this the liraglutide for weight management, the generic of Saxenda?" If yes, confirm your plan covers it and ask your prescriber whether it is the right request for you.

What brand Saxenda's discontinuation means for you

Novo Nordisk says it is discontinuing Saxenda, with last shipments to wholesalers in late January 2027. It says the decision is not because of any safety or quality concern. That's a shipping date, not the day every pharmacy runs out.

Novo's reason, in its own words: alternatives have become available and the number of people using Saxenda "has continued to decline" (NovoCare Saxenda page).

What it means in practice:

  • If you're starting now: ask about the generic. The shipment notice concerns brand Saxenda, and Cigna's initial employer-plan rule prefers the generic.
  • If you're on brand Saxenda: talk with your prescriber now about what comes next — the generic, or another medicine your plan covers. Don't wait for a pharmacy to say it's out.
  • If you're renewing: ask which continuation rules and benefits apply when your approval ends, and keep your records ready (see the checklist below).

Your prescriber decides any change in medicine. This page doesn't give switching or dosing instructions.

Where Saxenda sits on Cigna's drug lists

On Cigna's Standard 3-Tier drug list for coverage starting January 1, 2027, brand Saxenda is Tier 3, needs prior authorization, and is marked optional coverage. That's next year's list, not proof of today's benefit. Current California lists show why the exact list matters, and Express Scripts' 2026 National Preferred Formulary excludes brand Saxenda.

How to read the codes:

  • Tier 3 is the non-preferred brand tier, the highest cost-share on the national three-tier lists shown here. The California examples use their own lists.
  • PA means prior authorization.
  • OC means optional coverage. Cigna's own definition: "Most plans don't cover this medication, but some do."

That OC code is the whole story for many people. The Standard list's exclusions section names weight-loss medicines among the things plans "generally do not provide coverage for." Your plan's benefit design decides. A Cigna card alone doesn't tell you the answer.

Printed lists are short versions, and the generic isn't shown on the printed 2027 Standard list. Start with myCigna for your current coverage; ask the benefits team to resolve any conflict with your plan documents. We read more of Cigna's lists in our Cigna formulary comparison.

The right next step isn't the same for everyone. It depends on your state, plan and drug list, whether you'd stay on liraglutide or consider a weekly shot or a pill, whether you want an FDA-approved or compounded medicine, and your budget. The RX Index's Find My GLP-1 Path tool asks a few quick questions and shows a suggested provider path and pricing to review. It's free, takes about 2 minutes, and needs no signup. It doesn't check your Cigna coverage or copay — myCigna does that. Compounded medicines are not FDA-approved; the FDA does not review them for safety, effectiveness, or quality before marketing (FDA).

What Cigna requires to approve Saxenda

Under Cigna's standard liraglutide policies, adults need a BMI of 30 or more before any GLP-1 — or 27 or more with one listed health condition — plus at least 3 months of documented diet and behavior changes. Some employer plans buy weight-loss coverage as an add-on, and those use stricter versions. Your plan, not a website, decides which policy applies.

Cigna moved Saxenda and its generic into separate IP and benefit-override policies on September 1, 2026; the national-formulary policy has its own review date. Here are the four we read. They're not a menu; ask which one your plan uses.

BEO011, effective Sept 1, 2026A different add-on35+, or 27+ with 2 listed conditions—Initial employer-plan request: generic trial plus the documented inactive-ingredient exception

All four policies share these rules for adults:

  • Age 18 or older.
  • At least 3 months of diet and behavior changes, documented.
  • The medicine is used along with a reduced-calorie diet and behavior changes.
  • First approval: 4 months. Renewal: 1 year, if you've lost at least 4% of your starting weight and meet the other continuation requirements.

The listed conditions (same list in all four): high blood pressure, type 2 diabetes, high cholesterol (dyslipidemia), obstructive sleep apnea, cardiovascular disease, knee osteoarthritis, asthma, chronic obstructive pulmonary disease (COPD), metabolic dysfunction-associated steatotic liver disease/nonalcoholic fatty liver disease (MASLD/NAFLD), polycystic ovary syndrome (PCOS), and coronary artery disease.

"Starting weight" means before any GLP-1. BMI is body mass index, a number from your height and weight. Cigna uses your BMI from before you took any GLP-1, including Wegovy or Zepbound. Say you started at a BMI of 34, lost weight on another GLP-1, and you're at 28 now. Cigna looks at the 34. Ask your prescriber to put that earlier, dated weight in the request.

Teens 12–17: Cigna's policies use a BMI at or above the 95th percentile for age and sex, and renewal needs at least a 1% drop in BMI. The drug label also limits pediatric use to children 12 or older who weigh over 60 kg and have obesity. A teen's care starts with their own doctor; this guide is written for adults.

What your prescriber's office should send

Your prescriber needs records that match the policy your plan uses. Confirm that policy, then gather:

  • Your dated height and weight from before any GLP-1, plus a current weight.
  • Records of any listed condition, if your BMI is between 27 and the policy's top threshold.
  • Dates and notes showing at least 3 months of diet and behavior changes. A note like "patient counseled on diet" with no dates is weak; dated visits are strong.
  • The exact product: the generic of Saxenda, or brand Saxenda with any exception documentation required for that request.
  • For a renewal: your weights showing at least 4% loss from the starting weight.

The named Standard drug list says a review can take up to five business days after your doctor sends the information. Ask your plan which timeline applies to your request. More detail on paperwork: documents needed for GLP-1 prior authorization.

Not sure what to ask? Copy this

You can ask the plan to identify the benefit, product, and rule in one message. Send this through your plan's secure member channel, or read it on a call to the pharmacy number on your card:

Please check my pharmacy benefit for brand Saxenda and for the FDA-approved generic liraglutide that references Saxenda, for weight management. Does my plan include weight-loss medication coverage? Which drug list and which prior authorization policy apply to me, and what are their effective dates? If either product isn't covered, is the reason an excluded benefit, a non-preferred brand, prior authorization, missing records, or a pharmacy processing issue? Please send the rule and any denial in writing. Also confirm my cost for the prescribed quantity and days' supply, when any approval ends, and how to request review if the decision is wrong.

Write down the date, the rep's name, and a reference number.

The 4-month clock: how renewal works

Under the reviewed adult Cigna policies, Saxenda or its generic gets 4 months at first. Another year requires at least 4% weight loss from the policy's starting weight and the other continuation criteria. The label also uses a 4% adult response threshold, but its 16-week medical review is separate.

The reviewed adult weight-management policies for Wegovy, Zepbound, and Foundayo give 8 months up front. Liraglutide gets half that. So the clock starts fast.

The label's 16-week check is your prescriber's medical review (Saxenda prescribing information, revised 02/2026). Cigna's 4 months is an insurance approval window, counted as 30-day months. They're close but not the same date, so go by the end date on your approval letter.

Two things worth doing early:

  1. Get a dated weight at your prescriber's office before the 4 months are up.
  2. Put the renewal date on your own calendar, not just theirs. A lapse can mean paying full price for a fill.

Check where you stand

Enter your numbers below to compare the BMI and 4% weight-loss figures with Cigna's published adult rules. It doesn't check your benefit or the full approval criteria. Cigna decides, and your prescriber decides what's medically right.

Worked example: Say you're 5'6" and weighed 210 lb before any GLP-1. Your starting BMI is 33.9. That meets Cigna's standard rule (30+) and the BEO007 add-on rule (32+) on BMI alone, but not BEO011 (35+) unless you have two listed conditions. For the renewal weight-loss requirement, 201.6 lb is the 4% threshold — a loss of 8.4 lb. The other policy requirements still apply.

What Saxenda costs with Cigna

There's no single Cigna price for Saxenda. If your plan covers it and Cigna approves it, you pay your plan's cost-share for that tier, which depends on your deductible and plan design. Novo Nordisk's Saxenda savings offer ended in 2023, so it can't lower your cost now.

The Teva card only works when your commercial insurance covers Teva's liraglutide. It's not a cash discount, and Medicare, Medicaid, and other public-payer patients are not eligible. Maximum savings per prescription and per year apply, so some people pay more than $20. Maine and Maryland residents should follow the state-specific instructions in Teva's card.

Cigna's Standard drug list says that a fill without required approval won't count toward your deductible or out-of-pocket maximum. Ask your plan how any other cash purchase is handled.

Before you start: safety comes first

Insurance approval isn't a medical clearance. Saxenda's label says not to use it with a personal or family history of medullary thyroid carcinoma (a type of thyroid cancer) or multiple endocrine neoplasia syndrome type 2 (MEN 2), or after a serious allergic reaction to liraglutide or its ingredients. The label warns of potential harm to a developing baby and tells prescribers to discontinue it when pregnancy is recognized. Contact your prescriber if you become pregnant. The label does not recommend taking it with another GLP-1 (Saxenda prescribing information, revised 02/2026).

If you're under 18, pregnant or planning a pregnancy, have type 1 diabetes, or have a history of an eating disorder, start with your own clinician. This guide doesn't decide whether a medicine is right for you.

Should you ask about Wegovy or Zepbound instead?

On Cigna's Standard drug list for 2027, Wegovy and the Zepbound pen are Tier 2 (preferred brands), while Saxenda is Tier 3. The reviewed adult weight-management policies give Wegovy, Zepbound, and Foundayo 8 months at first and require 5% weight loss for renewal, plus other criteria; liraglutide gets 4 months and requires 4%. Which medicine is right for you is a decision for you and your prescriber.

The future-year tier list does not establish today's benefit. It lists the Wegovy brand without separating every formulation; confirm your exact product in myCigna. The table compares adult weight-management rules, not the drugs' other indications.

One useful head-to-head trial is STEP 8. Adults with overweight or obesity without diabetes on weekly semaglutide 2.4 mg injections lost about 15.8% of body weight on average at 68 weeks, versus about 6.4% on daily liraglutide 3 mg injections (JAMA, 2022). Both groups also received diet and activity counseling. These results are for the injections studied, not the pills. Averages aren't promises, and side effects and your health history matter as much as the number.

Some people do well on liraglutide and want to stay with it. That's a real choice, and with brand Saxenda ending, the generic is an option to discuss if you want to stay on liraglutide.

One honest limit: Ro's free insurance check covers Ozempic, Wegovy, and Zepbound pens. It doesn't check Saxenda or its generic. If you're staying on liraglutide, your own prescriber's office is the direct route. But if you're an adult with commercial coverage and you and your clinician are open to a weekly shot, Ro's team will contact your plan for you and email a written coverage report for Wegovy and Zepbound — free, and new Ro accounts get a $50 credit toward treatment (Ro checker, Oct 22, 2026). A report isn't an approval; it tells you where you stand before you ask.

If Cigna says no

A Saxenda denial can mean an excluded benefit, a product restriction, missing records, or a rule your request did not meet. Some problems can be corrected; others need a different coverage or treatment discussion. Get the reason in writing first, because the next step depends on it.

If Cigna says no

To ask for a review, follow the instructions and deadline in your denial letter. Cigna says the appeal process depends on your plan (Cigna appeals). Helpful next reads: how to appeal a GLP-1 denial, our appeal letter template, plan exclusion vs. prior authorization, and how formulary exceptions work. If your plan excludes weight-loss drugs, asking your employer to add coverage is a longer road, but it's the one that changes the answer.

If your plan excludes weight-loss drugs, an ordinary prior-authorization request does not create that benefit. Check whether an override applies, then discuss cash options if you want them. For adults with commercial insurance choosing a cash path, Ro lists these starting-dose prices as of October 22, 2026: $149 for 30 Wegovy tablets (1.5 mg), $149 for a 30-day Foundayo supply (0.8 mg), and $299 for a 28-day Zepbound KwikPen supply (2.5 mg). Later fills can cost more; these are not guaranteed ongoing-dose prices (Ro pricing; manufacturer supply terms for Wegovy, Foundayo, and Zepbound).

Ro's membership is separate: $39 for the first month, then $149 with monthly billing. It also advertises a 12-month plan at a $74 monthly equivalent, paid upfront after the first month. That advertised rate works out to $888 for 12 months; the actual charge is shown at checkout. Medication and membership are billed separately. Ro says the $39 is refunded if you're not eligible; otherwise, paid membership fees are nonrefundable. Cancel at least 48 hours before renewal to avoid the next charge (pricing, terms).

Medicare, Medicaid, and other Cigna plans

Cigna's commercial rules don't apply to Medicare or Medicaid. Medicare's $50 GLP-1 Bridge covers Foundayo, Wegovy, and the Zepbound KwikPen for eligible people — not Saxenda or its generic. Medicaid coverage depends on your state's own drug list.

  • Medicare: The Medicare GLP-1 Bridge runs July 1, 2026, through December 31, 2027, and its product list doesn't include liraglutide. Eligible people pay $50 for a one-month supply (28 or 30 days, depending on the drug); program eligibility and prior authorization still apply (Medicare.gov). If your Medicare plan came from Cigna, see our Cigna Medicare GLP-1 guide and our Medicare GLP-1 Bridge guide.
  • Medicaid: Check your state program's drug list. A commercial Cigna policy can't tell you what Medicaid covers.
  • Cigna individual and family (ACA) plans: Cigna says it will no longer offer Individual and Family medical plans beginning January 1, 2027 (Cigna's own notice). If that's your plan, arrange new coverage and check its rules; this notice does not apply to all Cigna employer plans.
  • Cigna's own employees: Check the current employee benefit documents and HR channel. Another employer's Cigna plan does not establish your benefit, and your employee plan does not establish theirs.
  • Changing jobs or plans: An old approval doesn't carry over automatically. Give your new prescriber your prior approval letter and your starting-weight records.

What we actually verified

Checked October 22, 2026: Cigna's Standard, Value, and Performance 3-Tier lists for coverage starting January 1, 2027 (updated September 1, 2026); the California Performance 3-Tier and Standard 4-Tier lists for coverage starting July 1, 2026; liraglutide policies IP0813, CNF1008, BEO007 and BEO011; and comparison policies IP0814, IP0815, IP0816 and IP0206. CNF1008 was reviewed June 17, 2026; the listed IP and BEO policies are effective September 1, 2026.

We also read Cigna's 2025 change notice, current member and appeals instructions, and individual/family plan notice; Express Scripts' October 1, 2026 exclusions; Novo Nordisk's shipment, savings, and package-price information; Teva's public copay terms; FDA generic approvals; current drug labels; the STEP 8 paper; CMS and Medicare.gov guidance; and Ro's pricing, terms, Saxenda, and checker pages. We checked the worked example and the calculator's formulas against those published rules.

What's provider-stated, not tested by us: Ro's checker scope, advertised prices, and treatment credit; Novo Nordisk's shipment plan; Teva's copay terms. We did not complete Ro intake or view an individual checkout. The $888 figure is $74 × 12, not a checkout quote.

Frequently asked questions

Can Cigna cover Saxenda if I don't have diabetes?

Yes, potentially. Saxenda is approved for weight management, not diabetes, and Cigna's liraglutide policies are built around weight-loss criteria like BMI. Your plan still has to include weight-loss drugs, and your records have to meet the policy's rules.

Is there a set list of criteria for Cigna to cover Saxenda?

Yes. Under Cigna's standard liraglutide policies, adults need a BMI of 30 or more before any GLP-1, or 27 or more with one listed condition, plus at least 3 months of documented diet and behavior changes. Add-on policies raise the BMI bar to 32 or 35, or require two conditions.

Does Cigna cover generic Saxenda?

Cigna's liraglutide policies address both brand Saxenda and its generic, but the policies do not create coverage under every plan. Cigna named the generic as the lower-cost choice when it moved the brand to a higher tier. Make sure the liraglutide on your plan is the weight-loss generic of Saxenda, not the generic of Victoza, which is a diabetes product.

My insurance stopped covering my Saxenda. What now?

Find out why in writing. If brand Saxenda was dropped, ask your prescriber about the generic. If your plan dropped weight-loss drugs altogether, ask HR about an add-on and compare FDA-approved cash options with your clinician.

Is Saxenda being discontinued?

Yes. Novo Nordisk says it's discontinuing Saxenda, with last shipments to wholesalers in late January 2027, and that it isn't because of safety or quality concerns. Pharmacies may have stock after that date, but don't count on it — talk with your prescriber about what's next.

How long does Cigna approve Saxenda for?

Under the reviewed adult policies, first approval lasts 4 months. Renewal can last 1 year if you've lost at least 4% of your weight from before any GLP-1 and meet the other continuation requirements.

Why did Cigna reject brand Saxenda but list liraglutide?

A product preference may explain it, but check which liraglutide and benefit the listing means. For initial treatment under IP0813's employer-plan rule, brand Saxenda requires a generic trial and documentation that an inactive-ingredient difference would cause a significant allergy or serious reaction.

What if my BMI is lower now because the medicine worked?

The reviewed Cigna policies use your BMI from before any GLP-1. Keep that dated starting weight in your records so your prescriber can show it for renewal.

Can I still use the Saxenda savings card?

No. Novo Nordisk closed the Saxenda savings offer to new enrollees on June 30, 2023, and benefits ended December 31, 2023. Teva lists a copay card for its generic, for people whose commercial plan covers it.

Does Medicare cover Saxenda?

The Medicare GLP-1 Bridge doesn't include Saxenda or its generic. Ask your plan whether it offers any supplemental weight-loss benefit; CMS recognizes that some plans have one. The Bridge lists Foundayo, Wegovy, and the Zepbound KwikPen for eligible people, subject to its own rules (CMS).

Can Ro check my Saxenda coverage for free?

No. Ro's free checker covers Ozempic, Wegovy, and Zepbound pens, not Saxenda. For Saxenda, myCigna and your prescriber's office are the right places to ask.

How long does Cigna take to decide?

The named Standard drug list says review can take up to five business days after your doctor's office sends the information. If records are missing, your prescriber can send more and ask for another review; check your plan's timeline and follow the request in myCigna.

Find My GLP-1 Path

The right GLP-1 provider isn't the same for everyone. It depends on whether you'll use insurance or pay cash, whether you want an FDA-approved or compounded medication, your preferred route (injection or oral), and your budget. Answer a few questions in The RX Index's Find My GLP-1 Path tool to see a suggested provider route and pricing to review before you choose. It does not check your plan's coverage or copay; confirm those in your insurer's member portal or with the pharmacy-benefits number on your card.

  • What it asks: your goals, how you plan to pay, and your treatment preferences
  • What you get: a suggested provider route and pricing information to review
